中厚皮片回植法腋臭根治及脱毛的临床研究

摘　　要：目的探讨采用中厚皮片回植法治疗腋臭及腋部脱毛的临床疗效。方法在每侧腋窝中部沿腋皱襞行3~4cm手术切口,在真皮中层至皮下脂肪层0.5cm深度,范围超出腋毛分布区1cm应用精细组织剪将包含大汗腺及毛囊的部分真皮及皮下组织彻底清除干净,创腔冲洗止血后用5.0丝线将切口严密缝合,应用烧伤外科方法行腋部打包固定,每侧腋部一块灭菌棉垫覆盖后自粘绷带加压包扎,术后7日拆除内部打包敷料,去除打包线,给予切口常规换药,14日行手术切口拆线。结果术后随访6个月~12个月,腋臭治愈率为100%,腋毛无生长。有2例男性患者单侧手术切口处有糜烂,后期出现疤痕增生,其余所有患者手术切口均一期愈合,无明显瘢痕,无皮片坏死等并发症。结论中厚皮片回植法腋臭根治及腋部脱毛具有疗效确切,无复发,脱毛彻底,并发症少等优点。Objective To explore the clinical efficacy of axillary osmidrosis and hair removel by medium thick skin graft .Methods A 3 - 4cm incision in the middle of each side axillary along the axillary fold was made. All tissues that contain large sweat glands were removed under middle dennis until the subcutaneous fat layer of 0.Sem depth, beyond the armpit peripheral nomal skin of lcm range by using fine tissue scissors. The surgical incisions were tightly sutured with 5.0 silk thread after washing and stopping bleeding. Bilateral axillary was fixed for 7 days with elastic adhesive bandage and then given routine treatment.Stitches were removed after two weeks. Results After 6 - 12 months follow-up, 2 cases of male patients underwent unilateral surgical incision erosion and then found scar hyperplasia, but no recurrence. All the other patients were healed well, no adverse reactions and hair growth. Conclusion The treatment of axillary osmidrosis and hair removel by medium thick skin graft have the advantage of definite curative effect,no recurrence, complete removal of hair, less complications and so on.
