奥司他韦联合痰热清治疗呼吸道合胞病毒肺炎的有效性和安全性

摘　　要：目的探讨奥司他韦联合痰热清注射液治疗呼吸道合胞病毒(RSV)肺炎的临床效果。方法选取2011年5月~2016年5月肇庆市第一人民医院儿科诊治的80例RSV肺炎患儿进行研究,采用随机数字表法分为痰热清组和奥司他韦组,每组各40例,两组均给予奥司他韦静脉滴注,痰热清组同时给予痰热清注射液治疗,疗程5~7 d,对比两组患儿的临床症状缓解及治疗效果。结果痰热清组患者的发热、咳嗽、喘憋、肺部啰音的消失时间、住院时间均短于奥司他韦组患儿,差异均具有统计学意义(P<0.05);治疗5 d后,痰热清组的呼吸频率(RR)、白细胞介素-2受体(IL-2R)值低于奥司他韦组患儿(P<0.05),每公斤潮气量(VT)、达峰时间比值(TPTEF/TE)、达峰容积比值(VPTEF/VE)、白细胞介素-2(IL-2)测定值高于奥司他韦组,差异均具有统计学意义(P<0.05);治疗5 d后,痰热清组的疗效优于奥司他韦组,差异有统计学意义(P<0.05)。结论奥司他韦联合痰热清注射液治疗RSV肺炎能有效缓解临床症状、改善肺功能、减轻炎症反应,提高临床疗效。Objective To discuss the clinical effect of oseltamivir combined with Tanreqing Injection in the treatment of respiratory syncytial virus （RSV） pneumonia. Methods 80 cases of children with RSV pneumonia in the First Peo- ple＇s Hospital of Zhaoqing from May 2011 to May 2016 were selected. They were randomly divided into Tanreqing group and osehamivir group,with 40 eases in each group. The two groups were both given oseltamivir intravenous. In addition, Tanreqing group was given Tanreqing Injection treatment for 5-7 d. The clinical symptoms relief and the therapeutic effect of the two groups were compared. Results Fever, cough, dyspnea, pulmonary rales disappeared time, hospitalization time of the Tanreqing group were all lower than those of osehamivir group, the differences were statisti- cally significant （P 〈 0.05）; after 5 d treatment, the respiratory frequency （RR） and interleukin -2 receptor （IL-2R） of Tanreqing group were both lower than those of osehamivir group （P 〈 0.05）, but tidal volume per kilogram （VT）, time to peak ratio （TPTEF/TE）, peak volume ratio （VPTEF/VE）, interleukin -2 （IL-2） were higher than those of oseltamivir group, the differences were statistically significant （P 〈 0.05）. After 5 d treatment, the treatment effect of Tanreqing group was better than that of oseltamivir group, the difference was statistically significant （P 〈 0.05）. Conclusion Oseltamivir combined with Tanreqing Injection in the treatment of RSV pneumonia can effectively relieve clinical symptoms, im- prove lung function, reduce inflammation, and improve clinical curative effect.
