超声心动图对移植后心脏形态、心功能及血流动力学早期变化的检测

摘　　要：目的:探讨超声心动图对移植心脏的早期变化检测价值。方法:应用超声心动图对10例原位心脏移植术后患者进行早期检测,分别为术后2、7、15、30天并描记心脏大小、心功能及血流动力学。结果:与术后2天比较,术后7、15、30天的LVDd均逐渐减小,RVDd也均逐渐减小,差异有统计学意义(P<0.05),与术后2天比较,术后7、15、30天的IVST、LVPWT数值均较高,差异有统计学意义(均为P<0.05),与术后2天比较,术后7、15、30天的EF、FS、SV均无明显改变,差异无统计学意义(均为P>0.05)。各时间点主动脉瓣前向血流比较差异无统计学意义(均为P>0.05)。E/A<1可用于粗略估计左室舒张功能,各时间点E/A均大于1,差异无统计学意义(均为P>0.05)。与术后2天比较,术后7天、15天、30天的PASP比较均逐渐增加,差异有统计学意义。结论:在心脏移植术后早期,超声心动可检测移植心脏的结果及功能情况,为临床医师提供诊疗依据。Objective： To investigate the value of echocardiography in early detection of transplanted heart. Methods： 10 patients with orthotopic heart transplantation were examined by echocardiography. The heart size, cardiac function and hemodynamics were recorded at day 2, day 7, day 15 and day after operation respectively. Results：The 2 day after surgery, after 7, 15, 30 days LVDd decreased, RVDd decreased, the difference was statistically significant （P 〈 0. 05）, and 2 days after surgery, postoperative 7, 15, 30 days of IVST and LVPWT values were higher, the difference was statistically significant （all P〈 0. 05）, and 2 days after surgery, postoperative 7, 15, 30 days of EF, FS and SV did not change significantly, the difference was not statistically significant （P 〉 0. 05）. There was no significant difference in the forward flow ofaortic valve at each time point （P 〉 0.05）. E/A〈1 can be used to estimate the left ventricular diastolic function, the E/A at each time point was more than 1, the difference was not statistically significant （P 〉 0. 05）. And after 2 days, after 7 days, 15 days, 30 days of PASP were gradually increased, the difference was statistically significant. Conclusion： echocardiography can be used to detect the outcome and function of the transplanted heart in the early stage of heart transplantation.
