摘 要:目的:评价祛瘀化痰通脉颗粒对麻醉比格犬心脏血流动力学及心肌耗氧量的影响。方法:采用开胸手术观察祛瘀化痰通脉颗粒对麻醉比格犬血流动力学及心肌耗氧量影响,比格犬30只,随机分为5组(每组6只),分别为正常组,合心爽组(5 mg·kg^(-1)),祛瘀化痰通脉颗粒高、中、低剂量组(2.0,1.0,0.5 g·kg^(-1)),经十二指肠给予相应药物,用多道生理记录仪分别测量动脉血压、心率、肢体标准Ⅱ导联心电图、冠状动脉血流量、心输出量、左室内压、左室内压上升最大速率。测定动脉血氧含量、冠状静脉窦血氧含量,计算心肌耗氧量、心肌耗氧指数及氧利用率等指标。结果:与正常组比较,祛瘀化痰通脉颗粒高、中剂量药后可以动物增加冠脉流量,降低冠脉阻力,祛瘀化痰通脉颗粒高、中剂量组药后15~60 min心输出量增加明显,与自身药前值及正常组比较均有统计学差异,祛瘀化痰通脉颗粒各剂量组药后均有增加左室作功的作用,中剂量给药后60 min见静脉血氧含量上升19%左右,心肌耗氧量降低约21%,与正常组及自身药前值比较均有统计学差异(P<0.05,P<0.01)。对心率、动脉血压、左室内压、左室内压上升最大速率、心肌耗氧指数等指标未见明显影响。结论:祛瘀化痰通脉颗粒可以改善比格犬心脏血流动力学指标,提高心脏泵血功能,增加冠脉血流量,同时还可以降低心肌耗氧量,从而提高了心脏的工作效率。Objective: To evaluate the effects of Quyu Huatan Tongmai granule( QHT) on the heart hemodynamics and myocardial consumption of oxygen in experimental beagle dogs. Method: In this experiment,thoracotomy was performed to observe the effects of QHT Granule on the heart hemodynamics and myocardial consumption of oxygen in experimental beagle dogs. Totally 30 beagle dogs were randomly divided into five groups:the normal group,diltiazem group( 5 mg·kg^-1),high-dose,medium-dose and low-dose QHT groups( 2. 0,1. 0,0. 5 g·kg^-1),with six in each group. They were given the corresponding drugs through duodenum. Multiple physiological recorders were adopted to measure arterial blood pressure,heart rate,standard Ⅱ ECG,coronary blood flow,resistance in coronary artery,cardiac output,left ventricular pressure,the maximal rate( dp / dtmax) of rise of left ventricular pressure,oxygen contents of the coronary artery and coronary vein( venous sinus),oxygen utilization,and oxygen consumption index. Result: Compared with control group,high-dose,medium-dose and low-dose QHT could significantly increase coronary artery blood flow and reduce coronary resistance. At 15-60 min after administration,high-dose and medium-dose QHT could increase cardiac output,with statistical differences from pre-administration value and normal group; QHT could improve the function of left ventricular; 60 min later after administration, medium-dose QHT could increase venous oxygen content by about 19% and decrease myocardial oxygen consumption by 21%,with statistical differences from normal group and pre-administration value( P〈0. 05,P〈0. 01). QHT showed no significant impact on heart rate,arterial blood pressure,left ventricular pressure,maximum rise rate of left ventricular pressure,myocardial oxygen consumption index. Conclusion: QHT can improve the cardiac hemodynamics and myocardial oxygen consumption in anesthetized beagle dogs.
