摘 要:目的观察云南地区2型糖尿病患者血清OPG/RANKL数值变动情况,研究病人的血清浓度和骨密度之间的相应关系.方法择取250例2型糖尿病患者,依照骨密度检测的数据把他们分成两组:糖尿病骨量对照组以及骨量降低组.2组都运用DEXA方法检测腰椎1-腰椎4(L1~L4)后前部位、腰椎1-4(L1~4)、股骨近端、全身、左前臂、右前臂的BMD数据,通过酶联免疫分析方法检测空腹状态下的血浆OPG/RANKL数值.结果(1)糖尿病骨量对照组OPG/RANKL数值明显高于骨量降低组,RANKL浓度显著低于骨量降低组;(2)在骨量降低组,依据简单线性相关分析(双变量相关性分析),了解到OPG和L1~L4、L1~4的BMD数据表现出正比例关系,OPG/RANKL和L1~L4、L1~4、左、右前臂的BMD数据也表现出正比例关系,RANKL和L1~L4、L1~4、左、右前臂的BMD数据表现出负比例关系.结论 OPG/RANKL体系对2型糖尿病患者体内出现骨质疏松症有着非常关键的作用.Objective To observe the change situation of serum OPG/RANKL of patients with type 2 diabetes in Yunnan district, and study the corresponding relationship between the patient's serum concentration and bone density. Me thod 250 cases of patients with type 2 diabetes were selected, divided into two groups according to the detected bone density data: the control group and bone mass reduction group. The control group was composed of T2 DM patients with normal bone mass. The BMD data of lumbar 1-lumbar 4(L1-L4) rear-front part, lumbar1-4(L1-4), proximal femur near-end, all parts of the body, left and right forearm were detected in both groups by using dual-energy X-ray absorption measures. The value of plasma OPG/RANKL without eating was measured by enzyme-linked immunoassay method. Re s ult(1) OPG/RANKL value in control group of diabetes bone mass was significantly higher than that in bone mass reduction group, and RANKL concentration was remarkably lower than that in bone mass reduction group.(2) According to the correlation analysis of double variant in low bone mass group, it was known that the OPG was in direct proportion to the BMD data of L1-L4 and L1-4. Besides,OPG/RANKL was also in direct proportion to the BMD data of L1-L4, L1-4, left and right forearm.RANKL was in inverse proportion to the BMD data of L1-L4, L1-4, left and right forearm. Conclusion OPG/RANKL system played a crucial role in the osteoporosis occurrence of patients with type 2 diabetes.
