摘 要:目的对患者深部分离的真菌进行检测和耐药性分析,了解患者深部真菌感染的现状,为临床感染性疾病提供病原学诊断和合理使用抗真菌药物的依据。方法患者血液、无菌体液培养、分离出的菌株用乳酸酚棉蓝染色直接镜检和培养相结合的方法进行鉴定及药敏试验。结果在1 829份血液和无菌体液标本中,分离丝状真菌35株,阳性率1.9%;其中以烟曲霉菌感染为主,检出前3位的科室为重症医学科(ICU)、肝胆外科和血液科;分离的菌株对药物的敏感性分别为氟康唑91.4%、5-氟胞嘧啶88.6%、伊曲康唑71.4%、两性霉素B100.0%。结论血液和无菌体液中分离的丝状真菌以烟曲霉菌最多见,药敏结果显示对伊曲康唑的耐药性增加。Objective To detect the deep fungi and analyze the drug resistance, it will provide the evidence lor the etiological diagnosis and using antithngal drugs rationally. Methods The deep fungi were identified from the blood and the sterile body fluid by the method of direct detection and culture of cotton blue staining. Results 35 strains of filamentous tung were identified from 1829 samples of blood and sterile specimens, the positive rate was 1.9%. The rnain strain was Aspergillus fumigatus, the top three departments were the intensive care unit (1CU), Department of hepatobiliary surgery and Department of Hematology. The sensitivity to antifungi drugs of the isolated fungi respectively were 91.4% for fluconazole; 88.6% for 5 fluorocytosine; 71.4% for itraconazole; 100.0% for amphotericin B. Conclusion The most common filamentous isolated from blood and sterile body fluid is aspergillus fumigatus, the results of drug sensitivity showed increased resistance to itraconazole.
