单侧椎弓根钉棒固定加单枚cage植入在腰椎退变融合术中的应用体会

摘　　要：目的比较单侧椎弓根钉棒与双侧钉棒固定、单枚cage椎间植骨融合术治疗腰椎退变的中短期临床疗效。方法本试验研究对象为2013年5月1日-2015年5月1日,入住东莞市第八人民医院骨外科腰椎退变的患者。回顾性统计行单侧钉棒固定患者18例,双侧32例。所有手术均由同一组医师施行。比较单侧椎弓根钉棒与双侧钉棒固定两组患者手术前后VAS评分,观察术后内固定有无松动或断裂、椎间融合器有无移位,出血量及手术时间的差异,通过以上参数评价手术效果。结果 (1)单侧椎弓根钉棒与双侧钉棒固定两组术后VAS评分明显提升并维持至末次随访,两种手术疗效差异无统计学意义(P>0.05)。(2)单侧椎弓根钉棒与双侧钉棒固定两组手术时间及出血量、术后引流量差异有统计学意义(P<0.01)。结论两种术式均有良好疗效,单侧椎弓根钉棒具有出血量少,手术时间短,残留慢性腰背肌肉疼痛少等优势。Objective To compare the short-term clinical effects of unilateral pedicle screw fixation and bilateral pedicle screw fixation and single cage interbody fusion in the treatment of lumbar degeneration. Methods The test objects were the degenerative lumbar spine patients from May 1, 2013 to May 1, 2015 trauma orthopedic branch of the Eighth People＇s Hospital in Dongguan City. Clinical data of 18 cases of unilateral nail rod fixation, and bilateral 32 cases were retrospectively analyzed. All operations were performed by the same group of physicians. Unilateral pedicle screw rod and double screw rod fixed in the two groups before and after operation with VAS score were observed postoperative internal fixation loosening or fracture and inter-vertebral fusion device with or without displacement, the differences in operation time and the volume of hemorrhage, through the above parameters to evaluate the effect of surgery. Results （1） Unilateral pedicle screw fixation and bilateral screw fixation in two groups after the VAS score was significantly improved and maintained to the last follow- up, there was no significant difference between the two types of surgery （P〉0.05）. （2） There was a significant difference between the two groups in the operation time, the amount of bleeding and the volume of postoperative drainage （P〈0.01）. Conclusion Two kinds of surgical procedures have good curative effect, the unilateral pedicle screw rod with less bleeding, shorter operation time, residual chronic low back muscle pain and other advantages.
