摘 要:目的分析不同程度夜间血氧饱和度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的临床特征及多导睡眠监测特点。方法选用OSAHS患者289例,按夜间最低血氧饱和度进行病情分组,其中轻度组69例,夜间最低血氧饱和度为85%~90%,中度组73例,夜间最低血氧饱和度为80%~85%,重度组147例,夜间最低血氧饱和度为<80%。分析不同夜间最低血氧饱和度OSAHS患者的临床特征包括年龄、体重指数(BMI)、颈围、Epworth嗜睡评分表(ESS)、氧减指数(ODI)、睡眠呼吸暂停指数(AHI)、暂停总时间、暂停最长时间、低通气总时间、低通气最长时间、睡眠结构中慢速眼球运动睡眠(NREM)1、2、3睡眠期及快速眼球运动睡眠(REM)睡眠期分别占总睡眠时间百分比。结果 3组患者的年龄、ESS评分、低通气总时间、低通气最长时间、REM期、NREM1期间无差异,BMI、ODI、AHI、暂停总时间、暂停最长时间、NREM2期、NREM3期间有明显差异,表现在重度组患者BMI、颈围显著高于轻度组与中度组,ODI、AHI、暂停总时间、暂停最长时间三组间均有明显差异,重度组NREM2期高于中、轻度组,重度组NREM3期少于中、轻度组。结论患者的BMI、颈围对患者最低血氧饱和度有影响;患者缺氧程度越重,则患者氧减指数及呼吸暂停指数越高,暂停总时间、暂停最长时间、NREM2期睡眠时间越长,而患者深睡眠NREM3期睡眠时间则越短。Objective In order to analyze the clinical characteristics and the characteristics of polysomnograpgy in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) at night. Methods 289 OSAHS patients were grouped according to the condition of lowest oxygen saturation,of which 69 cases of mild group,lowest oxygen saturation at 85% - 90 % ,moderate group in 73 cases, lowest oxygen saturation at 80 % - 85 %, 147 cases of severe group, lowest oxygen saturation at 〈80 %. Clinical characteristics of different lowest oxygen saturation in patients with OSAHS including age, body mass index ( BMI), neck circumference, Epworth sleep scale ( ESS), oxygen desaturation index ( ODI), apnea hypopnea index (AHI), total apnea time, the longest apnea time and total hypopnea time and the longest hypopnea time, non-rapid eye movements(NREM) stage 1, NREM stage 2,NREM stage 3 and rapid eye movements(REM) sleep the percentage of total sleep time period was analyzed. Results The 3 groups in age,ESS score, total hypopnea time and the longest hypopnea time,REM and NREM stage 1 during the between BMI and ODI, AHI, total apnea time, the longest apnea time, NREM stage 2 and NREM stage 3 there are obvious differences, performance was significantly higher than that of mild group and moderate group in severe group were BMI, ODI, AHI, neck circumference, total apnea time, the longest apnea time between the three groups were significantly different in severe group NREM stage 2 higher than in the mild group, severe group and mild group in a period of less than NREM stage 3. Conclusion The patients with BMI,neck circumference affects patients with the lowest blood oxygen saturation; hypoxia degree of patients with more severe patients, oxygen desaturation index and apnea index is higher, total apnea time, longest apnea time, NREM stage 2 longer,while the patients with deep sleep NREM stage 3 sleep time is short.
