2型糖尿病患者腰围身高比值与胰岛功能的相关性

摘　　要：目的探讨2型糖尿病(T2DM)患者腰围身高比值(WHt R)与胰岛功能的相关性。方法收集2016年1~11月于青岛内分泌糖尿病医院住院的T2DM患者180例,均行75 g OGTT或标准餐试验,分别测定空腹及服用糖水或馒头餐后0.5、1、2、3 h血糖及空腹C肽水平,以WHt R 0.5为切点分为非肥胖组和肥胖组,比较两组间身高、体重、血压、血脂、胰岛β细胞功能指数(HOMA-β)、胰岛素抵抗指数(HOMA-IR)等的异同,并行Pearson相关性分析和简单线性回归分析。结果肥胖组较非肥胖组体重、腰围、臀围、BMI、TG、UA、空腹C肽、HOMA-β、HOMA-IR升高,Hb A1c下降,差异均有统计学意义(P<0.05);Pearson相关分析显示,WHt R与年龄、体重、SBP、DBP、腰围、臀围、BMI、空腹C肽、HOMA-β、HOMA-IR均成正相关(P<0.05),与身高成负相关(P<0.01);简单线性回归分析显示,WHt R是HOMA-β和HOMA-IR的独立影响因素(P<0.01)。结论 WHt R是胰岛β细胞功能和胰岛素抵抗良好的预测因子。Objective To investigate the relationship between waist-height ratio （WHtR） and insulin function in patients withtype 2 diabetes mellitus （T2DM）.Methods One hundred and eighty cases of T2DM patients were enrolled in the study from January to November in 2016 in Qingdao Endocrine and Diabetes Hospital.They were all received 75 g oral glucose tolerance test or standard bread meal test.Plasma glucose and fasting C-peptide levels were measured at fast- ing,and after taking glucose or bread meal,plasma glucose and fasting C-peptide levels were measured at 0.5,1,2,3 h. With the cut point of WHtR at 0.5,180 patients were divided into non-obesitygroup and obesity group.The difference in height,weight,blood pressure,blood lipid,beta cell function （HOMA-β）,insulin resistance index （HOMA-IR） and so on were compared between two groups.The Pearson correlation analysis and simple linear regression analysis were then made.Results The obesity group had higher weight,WC,HC,BMI,TG,UA,fasting C-peptide,HOMA-β,HOMA-IR,but low- er level of HbAlc than non-obesity group,the differences were statistical significance （P〈0.05）.Pearson correlation anal- ysis indicated that, WHtR was positively correlated with age,weight,SBP,DBP,WC,HC,BMI,fasting C-peptide,HOMA-β, HOMA-IR （P〈0.05）,but negatively correlated with height （P〈0.01）.Simple linear regression analysis revealed that,WHtR was one of main influence factors on HOMA-β and HOMA-IR （P〈0.01）.Conclusion WHtR maybe a good predict fac- tor for β cell function and insulin resistance.
