摘 要:目的:探讨2型糖尿病阴阳两虚证与垂体-甲状腺轴的相关性。方法:将2型糖尿病患者499例分为阴虚热盛、湿热困脾、气阴两虚、阴阳两虚4型。测定患者静脉空腹血糖(FPG)、餐后2小时血糖(2 hPG)、糖化血红蛋白(HbA_(1c))、空腹血清C肽(FCP)、餐后2hC肽(2hPCP)、游离三碘甲状腺原氨酸(FT_3)、游离甲状腺素(FT_4)、促甲状腺激素(TSH)、甲状腺球蛋白(Tg)、甲状腺球蛋白抗体(Tg Ab)、甲状腺过氧化物酶抗体(TPOAb)水平。结果:阴阳两虚型患者的年龄、病程大于其余3型(P<0.001);气阴两虚型患者的年龄、病程大于阴虚热盛型(P<0.05)。气阴两虚型患者FPG、2h PCP高于阴阳两虚型(P<0.05);阴阳两虚型患者FT_3水平低于阴虚热盛型、湿热困脾型、气阴两虚型(P<0.01)。阴阳两虚型患者的FT_4水平显著低于气阴两虚型(P<0.01)。结论:甲状腺激素水平与2型糖尿病患者阴阳两虚证存在密切相关性。Objective:To explore the relationship between type 2 diabetes of deficiency syndrome of both Yin and Yang and pituitary thyroid axis. Methods:The 499 patients with type 2 diabetes were divided into four types of extreme heat due to Yin deficiency, damp-heat accumulating spleen, deficiency syndrome of both Qi and Yin, defi-ciency syndrome of both Yin and Yang. Indexes of venous fasting plasma glucose (FPG), 2-hour postprandial blood glucose(2 hPG), glycosylated hemoglobin (HbA1c ), fasting serum C peptide(FCP), postprandial 2hC peptide (2 hPCP), free three triiodothyronine (FT3), free thyroxine (FT4), thyroid stimulating hormone (TSH), thyroglobulin (Tg), thy-roglobulin antibody (TgAb) and thyroid peroxidase antibody (TPOAb) of the patients were determined. Results:The age and course of patients with deficiency syndrome of both Yin and Yang were significantly higher than those of the other three types (P〈0.001); The age and course of patients with deficiency syndrome of both Qi and Yin were greater than those of extreme heat due to Yin deficiency (P〈0.05). Moreover, FPG and 2 hPCP of the patients with deficiency syndrome of both Qi and Yin were higher than those of patients with Yin and Yang deficiency (P〈0.05);FT3 level in patients with Yin and Yang deficiency syndrome was remarkably lower than that of types of extreme heat due to Yin deficiency, damp-heat accumulating spleen, Qi and Yin deficiency(P〈0.01). In addition, FT4 level in patients with Yin and Yang deficiency was remarkably lower than that of the types of Qi and Yin deficiency (P〈0.01). Conclusion:There is a close correlation between the level of thyroid hormone and deficiency syndrome of both Yin and Yang in patients with type 2 diabetes.
