七氟醚吸入喉罩全麻与丙泊酚靶控输注全麻应用在无痛宫腔镜的费效比较

摘　　要：目的比较宫腔镜手术患者舒芬太尼复合七氟醚喉罩全麻和舒芬太尼复合丙泊酚靶控输注喉罩全麻的效果及费用。方法选择80例ASA Ⅰ~Ⅱ级择期无痛宫腔镜患者,随机分成舒芬太尼复合七氟醚全程吸入麻醉组(S组)和舒芬太尼复合丙泊酚靶控输注全麻组(P组)。记录意识消失时间、喉罩插入时间、自主呼吸恢复时间、苏醒时间、喉罩拔除时间、定向力恢复时间和麻醉后恢复室(PACU)停留时间;记录术中不良反应的发生情况;计算麻醉药物的费用。结果 S组患者意识消失时间、喉罩置入和拔除时间、定向力恢复时间和PACU停留时间均短于P组(P<0.05或P<0.01);两组患者不良反应发生率比较,差异无统计学意义(P>0.05);S组患者所用麻醉药物费用和单位每小时麻醉药物费用均低于P组(P<0.01)。结论舒芬太尼复合七氟醚吸入喉罩全麻是无痛宫腔镜手术患者最经济有效的麻醉方法。Objective To compare the cost-effectiveness of inhalation of sevofluarane and sulfentanil with laryngeal mask anesthesia（LMA） and target controlled infusion（TCI） of propofol and sulfentanil with LMA in painless hysteroscope. Methods Eighty cases of Grade Ⅰ and Ⅱ ASA were randomly divided into two groups： group of Sulfentanil and sevoflnarane （group S） and group of suifentanil and propofol （group P）. Time for loss of consciousness, time for LMA insertion, time for recovery of spontaneous breathing, emergence time,analepsia time, time for LMA removal, time for recovery of orientation, duration of stay in post anesthesia care unit （PACU） and adverse effects during surgery were recorded. The costs of anesthetics were calculated. Results Compared with group P, the time for loss of consciousness, time for LMA removal, time for recovery of orientation, and duration of stay in PACU were significantly shortened in group S, the total costs of anesthetics and the total cost of anesthetics per hour were decreased in groups P （ P 〈 0. 05 or P 〈 0. 01 ）. There was no significant difference in the incidence rates of adverse effects between the two groups （ P 〉 0. 05 ）. Conclusion Inhalation of sevofluarane and sulfentanil with LMA is the most cost-effective method of anesthesia for patients undergoing painless hysteroscopy.
