Quality Analysis of Death Medical Certificate Reporting in a Hospital

摘　　要：目的分析某院死亡医学证明书填报情况,对相关问题进行改进,提高死因填报质量。方法回顾性分析某医院2015年222份死亡死亡医学证明。结果 222份死亡医学证明中,93份填写有缺陷,缺陷率为41.89%;76份死因项填写不规范,不规范率为34.23%;25份死因编码不准确,编码不准确率为11.26%;9份未在规定时间内上报,迟报率为4.1%。结论建立健全死因登记报告管理制度,规范上报流程,落实部门职责,密切部门配合,确保人口死亡信息及时性、完整性、一致性,切实提高人口死亡信息登记管理水平。Objective To analyze the situation of the death medical certificate in a hospital, improve the related problems and the quality of death medical certificate fill in and report. Methods Retrospective analysis of 222 death medical certificate in a hospital in 2015. Results Among the 222 death medical certificate, 93 were filled with defects, the defect rate was 41.89%; 76 were filled in the non-standard item cause of death, the non-standard rate was 34.23%; 25 were the cause of death code is not accurate, coding inaccuracy rate was 11.26%; 9 were not reported within the specified time, delaying rate was 4.1%. Conclusion Establish and improve the management system of registration of death medical certificate, standardize the reporting process, implementat departmental responsibilities, carrying out close coordination between departments, ensure the timeliness, completeness and consistency of the death information of the population, effectively improve the registration and management level of population death.
