地塞米松处理外周中心静脉导管预防静脉炎的效果观察

摘　　要：目的 探讨两种药物溶液（地塞米松和生理盐水）对外周导管插入所致静脉炎的预防效果.方法 本研究选取2013年12月至2014年12月本院收治的需置入外周中心静脉导管（peripherally inserted central catheter,PICC）的患者200例,将患者随机分为治疗组和对照组,每组100例.治疗组置入PICC前用40 ml生理盐水加5 mg地塞米松浸泡导管,对照组置入PICC前用40 ml生理盐水浸泡导管,对比分析两组患者置入导管后静脉炎的发生率、发生时间以及其他并发症发生率.结果 置入PICC后,治疗组外周导管插入所致静脉炎发生率明显低于对照组（P〈0.05）;同时治疗组外周导管插入所致静脉炎发生时间明显晚于对照组（P〈0.05）.结论外周中心静脉导管置入时用地塞米松预处理对预防静脉炎有更好的疗效,且不产生其他副反应,值得临床借鉴.Objective To explore the preventive effect of two medicine solution （ dexamethasone and physio-logical saline ） on phlebitis caused by peripherally inserted central catheter Methods A total of 200 patients with the need of peripherally inserted central catheter in our hospital from December 2013 to December 2014 were randomly di-vided into treatment group and control group, 100 cases in each group. The catheter of the treatment group was soaked in 40 ml saline and 5 mg dexamethasone before the insertion of PICC, while the catheter of the control group was soaked in 40 ml physiological saline only. The incidence rate of phlebitis, time of occurance and incidence rate of other compli-cations after the insertion of PICC of the two groups were compared. Results After PICC were inserted, the incidence of phlebitis caused by PICC of patients of the treatment group was decreased significantly than that of the control group （P〈0. 05） . At the same time, the occurance time of peripheral phlebitis of patients of the treatment group caused by PICC was obviously later than that of the control group （ P〉0. 05 ） . Conclusions Peripherally inserted central catheter pretreated with dexamethasone was shown better effect to prevent phlebophlogosis without any other complications. It is worthy of reference for clinical.
