摘 要:目的探讨实时三维超声心动图(RT-3DE)技术评价心肌受损尿毒症病人左心室收缩功能及同步性的临床应用价值。方法选取尿毒症心肌受损病人(观察组)以及健康体检者(对照组)各30例,分别采用M型Teichholz法、双平面Simpson法与RT-3DE技术检测左心室收缩功能、同步性参数。结果 RT-3DE技术与M型Teichholz法、双平面Simpson法评估左心室射血分数(LVEF)均有良好相关性(r=0.874、0.930,P<0.05)。RT-3DE技术检测显示,观察组与对照组左心室舒张与收缩末容积、LVEF比较差异有显著性(t=-10.11~10.66,P<0.05);观察组左心室收缩同步性各参数均明显大于对照组,差异有显著性(t=5.50~21.35,P<0.05);观察组LVEF与各壁收缩同步性参数呈负相关(r=-0.749^-0.502,P<0.05)。结论 RT-3DE技术是评价尿毒症心肌损害病人收缩功能及同步性的一种无创、简便、准确的新方法。Objective To investigate the clinical value of RT-3DE in evaluation of left ventricular systolic function (LVSF) and synchrony in uremic patients with myocardial damage. Methods Thirty uremic patients with myocardial damage and 30 healthy individuals were enrolled in this study. LVSF and synchrony parameters were measured applying M-mode ultrasound of Teichholz method, biplane Simpson's method and RT-3DE method, respectively. Results RT-3DE method showed highly correlated with M-mode ultrasound of Teiehholz method and biplane Simpson' s method in assessment of left ventricular ejection fraction (LVEF) (r= 0.874,0.930;P〈0.05). RT-3DE detection showed that the differences in left ventricular diastolic volume (LVDV) and left ventricular end systolic volume (LVESV) as well as LVEF between the observation group and the control group were significant (t =--10.11-10.66 ,P〈0.05). In the observation group, LVEF were negatively correlated with wall systolic synchronization parameters (r= -0.749--0.502,all P〈0.05). Conclusion RT-3DE is a new method of non-invasive, simple and accurate in assessment of left ventricular systolic function and synchrony in uremic patients with myocardial damage.
