摘 要:目的了解妊娠早期妇女血清25羟维生素D(25(OH)D)水平,探讨其与甲状腺功能减退症的相关性。方法测定248例妊娠早期妇女血清25(OH)D、促甲状腺激素(TSH)、游离甲状腺素(FT4)、三碘甲状腺原氨酸(FT3)和甲状腺过氧化物酶抗体(TPOAb)的水平。结果妊娠早期妇女25(OH)D水平为5.35~44.33μg/L,中位数为13.14μg/L。维生素D缺乏、不足、充足各组的构成比分别为88.71%(220/248)、8.87%(22/248)与2.42%(6/248)。不同维生素D营养状态组间血清TSH、FT_4、FT_3水平和甲状腺功能减退症患病率比较,差异均无显著性(P>0.05)。甲状腺功能减退症各组与甲状腺功能正常组间不同25(OH)D水平及频数分布比较,差异均无显著性(P>0.05)。血清25(OH)D水平与TSH、FT_4、FT_3、TPOAb间均无相关性(P>0.05)。日照充足季节组血清25(OH)D水平高于日照不足季节组,差异有显著性(Z=-10.54,P<0.05)。结论妊娠早期妇女25(OH)D缺乏相当普遍;季节对妊娠早期妇女25(OH)D水平有一定影响。Objective To investigate the serum level of 25(OH)D in early pregnancy women and its correlation with hypothyroidism. Methods A detection of serum 25(OH)D,thyrotropin(TSH),free thyroxine(FT4),free triiodothyronine(FT3),and thyroid peroxidase antibody(TPOAb)was conducted in 248 cases of early pregnancy. Results The level of serum 25(OH)D in early pregnancy women was 5.35-44.33μg/L,the median being 13.14μg/L.The constituent ratios of each group with vitamin D deficiency,lack of vitamin D,and vitamin D sufficiency were 88.71%(220/248),8.87%(22/248)and 2.42%(6/248),respectively.The differences in prevalence of hypothyroidism among serum levels of TSH,FT4 and FT3 in vitamin D nutritional status were not significant(P〉0.05).There were no significant differences between hypothyroidism groups and normal thyroid function group in terms of 25(OH)D and frequency distribution(P〉0.05).The level of serum 25(OH)D was not correlated with TSH,FT4,FT3,and TPOAb(P〉0.05).The serum level of 25(OH)D in sunny season group was higher than that in lack of sunshine season group(Z=-10.54,P〈0.05). Conclusion Deficiency of 25(OH)D is common in early pregnancy women,and season has a certain influence on its level.
