血流感染铜绿假单胞菌耐药性分析

摘　　要：目的了解血流感染铜绿假单胞菌的临床分布及耐药性。方法收集我院2010—2015年血培养中分离出的铜绿假单胞菌,采用VITEK-2compact全自动药敏鉴定分析仪对菌株进行鉴定及药敏试验,采用WHONET 5.6软件进行药敏结果分析。结果 6年间我院共检出铜绿假单胞菌93株,占分离细菌总数的4.52%。感染科室分布以ICU为主,占26.88%。药敏试验结果显示,分离出的铜绿假单胞菌对哌拉西林耐药率较高,对阿米卡星耐药率最低,其对哌拉西林、哌拉西林-他唑巴坦、头孢他啶、头孢吡肟、亚胺培南、阿米卡星、庆大霉素、妥布霉素、环丙沙星、左旋氧氟沙星的耐药率分别为40.4%、26.9%、21.5%、19.4%、38.7%、5.4%、29.0%、29.0%、35.5%和34.4%,未发现对多黏菌素B耐药菌株。多重耐药株的总检出率为46.23%。结论铜绿假单胞菌在血流感染中占重要地位,血培养临床分离出的铜绿假单胞菌株对常用抗菌药物耐药性较严重。Objective To understand the clinical distribution and drug resistance of Pseudomonas aeruginosain bloodstream infection. Methods Pseudomonas aeruginosa that isolated from blood culture done in our hospital between 2010and2015 were collected.Appying VITEK-2compact system,the isolates were identified and drug sensitivity test done.Drug sensitivity analysis was carried using WHONET 5.6. Results In six-year time,93 strains of Pseudomonas aeruginosa were isolated,accounting for 4.52% of the total number of isolates.For the distribution of infection,mostly was（26.88%）found in ICU.The results of drug sensitivity test showed that the resistance rate of Pseudomonas aeruginosaisolated to piperacillin was higher,and that to Amikacin was the lowest.The resistance rate of the strain isolated to piperacillin,piperacillin/tazobactam,ceftazidime,cefepime,imipe-nem,amikacin,gentamicin,tobramycin,ciprofloxacin,and levofloxacin was 40.4%,26.9%,21.5%,19.4%,38.7%,5.4%,29.0%,29.0%,35.5% and 34.4%,respectively.The strains resistant to Polymyxin B were not found.The total detection rate of multi-drug resistant strains was 46.23%. Conclusion Pseudomonas aeruginosa plays an important role in bloodstream infection.The strains isolated from blood culture of this study showed drug resistant to commonly used antibiotics was serious.
