门诊癌痛病人疼痛及身心症状的调查分析

摘　　要：目的:通过对门诊癌痛病人进行系统的随访,调查分析癌症病人的疼痛程度、躯体及心理症状,以便更好地控制癌痛并改善病人的躯体及心理症状。方法:对2015年7月至2016年7月在北京大学肿瘤医院疼痛门诊就诊的癌痛病人,分别于首次就诊3天内、2周后、4周后进行三次随访,采用疼痛数字评分表和中国版记忆症状评估简表对癌症病人的疼痛程度、躯体及心理症状进行评估。结果:102例癌痛病人参加随访,98例病人完成两次随访,49例病人完成三次随访。首次随访中65.7%的病人在调查当时诉中、重度疼痛。在之后的每次随访中,疼痛评分均比前一次随访减少。另外,首次随访中癌症病人躯体及心理症状的发生率较高,位于前10位的症状除疼痛外,还有感到烦躁、缺乏精力、感到担心、睡眠有困难、食欲不振、感到不快乐、恶心反胃、便秘及难以集中精神。结论:疼痛随访能够帮助临床医生及时发现并处理疼痛治疗中存在的问题,对癌症病人的疼痛管理起到了积极作用。Objective： To investigate the pain, physical and psychological symptoms in outpatients with cancer pain through a follow-up program to gain some evidence for better pain and symptom management. Methods： The outpatients with cancer pain in the pain clinic in Beijing Cancer Hospital were asked to participant in the follow-up program from July 2015 to July 2016. Three times follow-ups were conducted within 3 days, two weeks and four weeks after the first visit. Numeric Rating Scale and The Chinese Versions of the Memorial Symptom Assessment Scale Short Form were used to assess the pain, physical and psychological symptoms. Results： 102 patients with cancer pain participated in the follow-up, 98 completed 2 follow-ups, and 49 completed 3 follow-ups. We found that 65.7% of the patients reported moderate to severe pain at the first follow-up. The pain intensity at each follow-up was reduced compared with the previous one. The prevalence of physical and psychological symptoms was high in the patients with cancer pain in the first follow-up. The top ten symptoms besides pain were feeling irritable, lack of energy, worrying, difficulty sleeping, lack of appetite, feeling sad, nausea, constipation, and difficulty concentrating. Conclusion： Pain follow-ups for outpatients with cancer pain are helpful to find out and solve the problems in pain control, which played a positive role in pain management.
