实时三维超声心动图评价早期心尖肥厚型心肌病的左心室收缩同步性

摘　　要：目的应用实时三维超声心动图(RT-3DE)技术评价早期心尖肥厚型心肌病(PAHCM)患者左心室收缩的同步性。方法选取26例PAHCM患者作为PAHCM组,同时选取26名性别相匹配的健康自愿者作为健康对照组。于心尖四腔心切面应用全容积显像方式采集RT-3DE图像,获得左心室舒张末期容积(EDV)、收缩末期容积(ESV)和左心室射血分数(LVEF),同时获得左心室16节段达到收缩末期最小容积时间的标准差(Tmsv16-SD)、时间的差值(Tmsv16-Dif)、经心率校正后的Tmsv16-SD(Tmsv16-SD%)和经心率校正后的Tmsv16-Dif(Tmsv16-Dif%)。结果与健康对照组比较,PAHCM组EDV、ESV之间差异均有统计学意义(P均<0.05),而LVEF之间差异无统计学意义(P均>0.05)。与健康对照组比较,PAHCM组Tmsv16-SD,Tmsv16-Dif,Tmsv16-SD%,Tmsv16-Dif%明显增加,差异有统计学意义(P<0.05)。结论 RT-3DE能准确反映PAHCM患者左心室收缩的同步性,为临床评估以及尽早干预治疗提供了一种检测手段。Objective To assess the left ventricular systolic synchrony in patients with pre-apical hypertrophic cardiomyopathy （PAHCM） using real-time three-dimensional echocardiography （RT-3DE）.Methods Twenty-six patients with PAHCM and twenty-six healthy volunteers were enrolled in the study.The apical four-chamber view of left ventricular was acquired by RT-3DE.The left ventricular volume-time curves were analyzed quantitatively.The left ventricular end-diastolic volume （EDV）, end-systolic volume （ESV）, left ventricular ejection fraction （LVEF） were measured.The standard deviation of the time to the point of minimal regional systolic volume from 16 segments （Tmsv16-SD）, the maximal difference of the time to the point of minimal regional systolic volume from 16 segments （Tmsv16-Dif）, the Tmsv16-SD adjusted by R-R interval （Tmsv16-SD%） and the Tmsv16-Dif adjusted by R-R interval （Tmsv16-Dif%） were measured at the same time.Results There are significant difference in EDV and ESV between normal control group and PAHCM group （P〈0.05）.There is no significant difference in LVEF between normal control group and PAHCM group （P〉0.05）.Compared with normal control group, Tmsv16-SD, Tmsv16-Dif, Tmsv16-SD% and Tmsv16-Dif% in PAHCM group were significantly increased.There are significant difference in these characters between normal control group and PAHCM group （P〈0.05）.Conclusion RT-3DE can accurately reflect the left ventricular systolic synchrony in patients with PAHCM, and provide a method for clinical evaluation and early intervention.
