不同踝肱指数病程阶段血栓闭塞性脉管炎患者血浆D-二聚体及假性血友病因子变化

摘　　要：目的探讨不同踝肱指数(ABI)病程阶段血栓闭塞性脉管炎(TAO)患者血浆中D-二聚体(D-D)与血管性假血友病因子(vWf)的差异性表达。方法选择我院就诊的TAO患者30例,测量患者ABI并抽取健康人血浆和患者治疗前、治疗后7d、14d血浆,酶联免疫吸附试验检测2组vWf以及D-D的不同,Pearson相关性分析vWf以及D-D与ABI的相关性。结果 TAO患者血浆中vWf高于健康对照组(t=26.189,P<0.01),D-D含量也高于健康对照组(t=84.012,P<0.01)。TAO患者经抗凝扩管以及激素综合治疗后7d与14d后vWf以及D-D含量均下降,与术前比差异均有统计学意义(P<0.05),vWf含量与ABI呈负相关(r=-0.40,P<0.05),D-D含量与ABI呈负相关(r=-0.45,P<0.05)。结论 TAO患者血浆vWf以及D-D含量均高于健康对照组,并且病情严重程度与vWf以及D-D含量成负相关,vWf以及D-D含量变化有望为不同病程阶段TAO诊断提供依据。Objective To explore the differential expression of serum vWf and D-D in different ankle-brachial index（ABI）stages of TAO patients.Methods Thirty patients with TAO in our hospital were involved into TAO group.Thirty normal people were taken into control group and the plasma was checked.The ABI of TAO group was measured.The plasma of TAO group was checked before treatment,7days and 14 days after treatment.ELISA was used to detect the difference between patients′vWf and D-D.Pearson correlation was used to analyze the correlation between vWf,D-D and ABI.Results The serum vWf and D-D in the TAO group was higher than those in the control group（t=26.189,P〈0.01,t=84.012,P〈0.01）.After treated with flaring anticoagulant therapy and hormone synthesis.The vWf and D-D level in the TAO group were decreased more than before treatment.The difference was statistically significant（P〈0.05）.The vWf content and D-D content were negatively correlated with ABI（r=-0.40,P〈0.05;r=-0.45,P〈0.05）.Conclusion The vWf and serum DD in TAO patients are higher than those in the control group.The severity of the disease is negatively correlated with vWf and D-D content.The change of vWf and D-D content is expected to provide important information for the different stages of TAO diagnosis.
