股骨近端抗旋髓内钉和锁定钢板治疗老年股骨转子间骨折患者综合疗效评价

摘　　要：目的探讨股骨近端抗旋髓内钉和锁定钢板治疗老年股骨转子间骨折患者综合疗效。方法选取70例于2014年4月至2016年2月我院诊治的股骨转子间骨折老年患者,按照随机数字表法分为2组,每组各35例。A组给予股骨近端抗旋髓内钉治疗,B组采用锁定钢板治疗。观察2组患者的手术时间、术中出血量、切口长度、术后引流量、住院天数、骨折愈合时间差异,比较2组患者的术后Harris髋关节评分及术后并发症发生情况。结果 A组患者的手术时间、术中出血量、切口长度、术后引流量、住院天数均显著低于B组(P<0.05);2组患者的髋关节功能评分及术后并发症比较差异无统计学意义(P>0.05)。结论股骨近端抗旋髓内钉治疗老年股骨转子间骨折较锁定钢板治疗综合疗效更加显著,具有手术时间及住院时间短,术中出血量和术后引流量少,切口长度小等优势。Objective To discuss the comprehensive therapeutic effect of proximal femoral anti-rotation intramedullary nail and locking plate in the treatment of elderly patients with intertrochanteric fractures.MethodsSeventy cases of elderly patients with intertrochanteric fractures treated in our hospital from April 2014 to February 2016 were selected and divided into A and B groups by a random digital table,with 35 cases in each.The A group was treated with anti-rotation intramedullary nail,while the B group was treated with locking plate.The differences in the operation time,intraoperative blood loss,incision length,postoperative drainage volume,hospital stay,fracture healing time between the two groups were observed,and the Harris hip scores and complications after operation were compared between the two groups.Results The operation time,intraoperative blood loss,incision length,postoperative drainage volume,hospital stay and fracture healing time of patients in the A group were significantly lower than those in the B group（P〈0.05）.The differences in the Harris hip scores and complications after operation were not statistically significant between the two groups（P〉0.05）.Conclusion The comprehensive therapeutic effect of proximal femoral anti-rotation intramedullary nail in the treatment of elderly patients with intertrochanteric fractures is more significant,with advantages as short operation time and hospitalization time,less blood loss during operation and less postoperative drainage volume,short incision length.
