曲美他嗪对老年慢性心力衰竭患者心功能及心率变异性的影响

摘　　要：目的探讨曲美他嗪对老年慢性心力衰竭患者心功能及心率变异性的影响。方法选择老年慢性心力衰竭患者64例,按简单随机法分为治疗组和对照组各32例,对照组采用常规治疗,治疗组在此基础上加用曲美他嗪20 mg,tid,治疗3个月,比较两组治疗前后的心功能及心率变异性。结果治疗组总有效率高于对照组。两组治疗后左室射血分数、6分钟步行距离均高于治疗前,全部正常RR间期的标准差(SDNN)、SDNN的均值(SDNNi)、每5 min RR间期均值的标准差(SDANN)、正常相邻RR间期差值的均方根(r MSSD)和正常相邻RR间期差值>50 ms的百分比(PNN50)亦显著高于治疗前,且治疗组SDNN、SDANN、r MSSD、PNN50均较对照组增高,差异均有统计学意义(P<0.05)。结论曲美他嗪可以改善慢性心力衰竭患者的心功能,同时改善自主神经功能。Objective To examine the effects of Trimetazidine on heart rate variability in patients with chronic heart failure （CHF）. Methods Eighty CHF patients were randomized divided into treatment or control group,40 in each group. The both groups were given conventional treatment, while the treatment group were added Trimctazidine orally,20 mg each time and three times each day for three months. Results The total effective rate in treatment group was higher than that in control group. There were significant differences in left ventricular ejection fraction （LVEF） , standard deviation of normal RR intervals （SDNN） , standard deviation of the mean RR intervals for all 5-minute segments （ SDANN）, root mean square of the successive differences （RMSSD） of normal RR interval, mean of the standard deviation of all RR intervals for all 5-minute segments （SDNNi） , and percentage of successive normal sinus RR intervals 〉50 ms （PNNS0） in the both groups before and after treatment （P 〈 0. 05 ）. After treatment,LVEF, SDNN,SDANN, RMSSD and PNNS0 in the treatment group differed from those in the control group （P 〈 0. 05 ）. Conclusion Trimetazidine may improve cardiac function and autonomic nerve function in CHF patients.
