血清HSP70和TNF-α表达水平对宁夏回汉族原发性高血压的影响

摘　　要：目的探讨血清热应激蛋白70(HSP70)和肿瘤坏死因子α(TNF-α)表达水平与宁夏回汉民族原发性高血压(EH)发病的关系。方法按照纳入与排除标准,从2011年10月至2012年10月期间在宁夏医科大学附属医院和吴忠市人民医院进行健康检查以及在内分泌科住院的人群中,选择229例无血缘关系的原发性高血压患者为病例组,600例无血缘关系的健康人为对照组。结果 EH组血清HSP70表达水平明显高于对照组(P<0.05)。调整性别、年龄和民族因素后,HSP70高水平人群EH的发病风险是低水平人群的2.163倍。HSP70表达水平与收缩压(SBP)呈正相关(P<0.001),与舒张压(DBP)无相关性(P<0.05),TNF-α表达水平与SBP、DBP均呈正相关(均P<0.05)。回汉民族间血清TNF-α表达水平差异均有统计学意义(均P<0.05)。不同TNF-α表达水平间SBP、DBP水平差异均有统计学意义(均P<0.05)。结论血清HSP70表达水平与原发性高血压发病密切相关,随着HSP70表达水平的增高,EH的发病风险也明显增高;TNF-α表达水平可能是原发性高血压的危险因素,也可能与种族遗传有关。Objective To investigate the influence of the expression level of tumour necrosis factor alpha （ TNF - α） and heat stress protein 70 （ HSP70 ） in serum on the incidence of essential hypertension （EH） in Ningxia Hui Han nationality. Methods According to inclusion and exclusion criteria, we chose 229 patients with primary hypertension who were treated by the department of endocrinology in the affiliated hospital of Ningxia medical university and Wuzhong city people＇s hospital from October 2011 to October 2012 as cases group and chose 600 healthy people during health checkups as a control group. Results The expression level of HSP70 in the serum of EH group was significantly higher than that of control group（ P 〈 0.05 ）. After adjustment for gender, age and ethnic, the risk of essential hypertension for high HSP70 expression level people was 2. 163 times than lower expression people. The expression level of HSP70 in serum had a positive correlation with SBP（ P 〈 0. 001 ） and no correlation with DBP（ P 〈 0.05 ）. The expression level of TNF -α in serum had a positive correlation with SBP, DBP（ all P 〈 0. 05 ）. The expression level of TNF - α in serum was signifieantly different between Hui and Han in the ease group, control group and total survey population （ all P 〈 0.05 ）. There were significant differences in the levels of SBP, DBP between different expression levels of TNF -α in the case group, control group and total survey population （ all P 〈 0.05 ）. Conclusion The expression level of HSP70 in serum is closely related to the pathogenesis of essential hypertension, with the increase of expression level of which, the risk of essential hypertension increases obviously. The expression level of TNF - α in serum may be a risk factor for essential hypertension and its expression level may be associated with racial inheritance.
