摘 要:目的探讨血管紧张素转化酶基因I/D(ACEL/D)多态性与白癜风的关系。方法检索PubMed、CochraneLibrary、中国学术期刊全文数据库、维普数据库和万方数据库,按纳入、排除标准纳入研究,评价纳入研究质量,并采用Stata12.0进行定量分析。结果最终纳入9篇病例对照研究。Meta分析结果显示,总人群的ACEI/D中基因型(DD+II)VS.DI(OR=0.759,95%CI:0.643~0.896,P:0.001),(DD+DI)VS.II(OR=1.523,95%CI:1.153~2.011,P=0.003)与白癜风易感性之间有相关性。亚组分析显示,亚洲人中基因型DVS.I(OR=1.381,95%C1:1.054—1.810,P=0.019);DDVS.II(OR=1.830,95%:1.110~3.017,P=0.018);(DD+II)VS.DI(OR=0.814,95%C1:0.667~0.994,P=0.043);(DD+ID)VS.II(OR=1.690,95%CI:1.147~2.489,P:0.008);与白癜风发生有关。阿拉伯人群基因型(DD+II)VS.DI(OR=0.545,95%C1:0.354~0.840,P=0.006)与白癜风发生有关。通过Begg’S检验判断发表偏倚,所有基因类型均不存在任何发表偏倚。结论ACEI/D基因多态性可能与白癜风易感性有关。Objective To investigate the relationship of angiotensin converting enzyme ( ACE I/D) gene LiD polymorphism with vitiligo. Methods PubMed, the Cochranc Library, China academic journal full-text database and VIP database and Wan fang database were searched according to the inclusion and ex- clusion criteria, and the quality of included studies was assessed. The data were quantitatively analyzed by Stata 12. 0 software. Results Nine articles were included. Meta analysis results showed that the suscepti- bility of vitiligo difference was statistically significant in the total population of ACE LiD genotype (DD + II) vs. DI (OR=0.759, 95% CI: 0.643 -0.896, P:0.001), the (DD+DI) vs. II (OR=1.523, 95% CI: 1. 153 -2. 011, P =0. 003). The genotype subgroup analysis showed the D vs. I (OR = 1. 381, 95% CI: 1. 054 -1. 810,P=0. 019) , DDvs. II (OR=1.830, 95% CI:1.110-3.017, P=0.018), (DD+ lI) vs. DI ( OR = 0. 814, 95% CI:O. 667 - 0. 994, P = 0. 043), and the ( DD + ID) vs. II ( OR = 1. 690, 95% CI: 1. 147 - 2. 489, P =0. 008) in Asian. The difference was statistically significantly related to viti- ligo. Arab genotype ( DD + II) vs. DI ( OR =0. 545, 95% CI:O. 354 - 0. 840, P =0. 006) was associated with vitiligo occurrence. Begg's inspection publication bias analysis showed all genetic types did not exist any publication bias. Conclusions The ACE gene I/D polymorphism may be associated with vitiligo sus- ceptibility.
