摘  要:目的研究婴幼儿喘息性疾病呼出气一氧化氮(FeNO)水平及其与特异性免疫球蛋白E(sIgE)、IgE的相关性。方法选择2012年5月~2015年6月于东莞康华医院就诊的65例婴幼儿喘息患儿作为观察组,按照1∶1匹配原则选择同期体检的健康儿童65例作为对照组,测定FeNO水平,采集血清测定总IgE(tIgE)和尘螨、蛋清、牛奶sIgE以及白细胞介素-4(IL-4)、白细胞介素-10(IL-10)、干扰素-γ(IFN-γ)含量。结果观察组FeNO水平均显著高于对照组(P<0.01),血清tIgE以及尘螨、蛋清、牛奶sIgE含量均显著高于对照组(P<0.01),血清IL-4、IL-10、IFN-γ含量均明显高于对照组(P<0.01)。FeNO水平与血清tIgE和尘螨、蛋清、牛奶sIgE及IL-4、IL-10、IFN-γ含量呈正相关(r=0.772、0.602、0.685、0.591、0.714、0.693、0.753,P<0.05)。结论婴幼儿喘息性疾病的FeNO显著升高,接触变应原所引起的sIgE、IgE升高是造成FeNO升高的相关因素,而FeNO的升高又会促进炎症介质的分泌、加重气道炎性反应。Objective To study correlation of exhaled nitric oxide (FeNO) detection and sIgE, IgE in infants with asthmatic disease. Methods 65 infantile with asthmatic disease in Dongguan Kanghua Hospital during May 2012 to June 2015 were enrolled as observation group, 65 cases healthy children received physical examination during the same period were enrolled as control group. FeNO level was determined. Serum was collected and total IgE and dust mite, egg white, milk sIgE and IL-4, IL-10, IFN-γ/contents were determined. Results FeNO level of observation group was significantly higher than that of control group (P 〈 0.01). Serum total IgE content and dust mites, egg white, milk sIgE contents in observation group were significantly higher than those in control group (P 〈 0.01). Serum IL-4, IL-10, IFN-γ/contents in observation group were significantly higher than those in control group (P 〈 0.01). FeNO level was positively correlated with serum total IgE content and dust mites, egg white, milk sIgE contents, serum IL-4, IL-IO, IFN-γ/contents (r = 0.772, 0.602, 0.685, 0.591, 0.714, 0.693, 0.753, P 〈 0.05). Conclusion FeNO of infants with asthmatic disease is significantly higher. Increase of sIgE, IgE contents caused by allergens is related factors with increase of FeNO level, and the increase of FeNO will promote the secretion of inflammatory mediators and aggravate airway inflammation.
