针刺八穴配合盆底肌肉训练治疗前列腺气化电切术后并发尿失禁76例临床观察

摘　　要：目的观察针刺八髎穴配合盆底肌肉训练治疗前列腺气化电切术后尿失禁患者的临床效果。方法选取2012年7月~2016年12月陕西中医药大学附属医院、咸阳市第一人民医院行前列腺气化电切术后尿失禁的患者共76例,使用随机数字表将其分为治疗组与对照组,各38例。对照组行常规治疗,治疗组行针刺八髎穴配合盆底肌肉功能训练,连续治疗1个月。观察两组患者治疗前后症状评分变化,比较两组临床疗效。结果两组患者治疗后尿急、有尿意是否能忍到厕前、夜尿间隔时间、夜尿次数、24 h漏尿次数、排尿诱发因素等症状评分均较治疗前明显降低(P<0.05),且治疗组治疗后上述各项症状评分均明显低于对照组,差异有统计学意义(P<0.05)。治疗组总有效率为94.7%,对照组总有效率为78.9%,差异有统计学意义(P<0.05)。结论前列腺气化电切术后尿失禁通过针刺八髎穴配合盆底肌肉训练治疗效果确切,操作简便,是一种治疗前列腺气化电切术后尿失禁的好方法,值得大力推广应用。Objective To observe the clinical effect of acupuncture at Baliao cooperated with pelvic muscles exercise in the treatment of urinary incontinence after electro-vaporization of prostate. Methods Total 76 cases with urinary in- continence after electro-vaporization of prostate in the Affiliated Hospital of Shaanxi University of Chinese Medicine and the First People＇s Hospital of Xianyang City from July 2012 to December 2016 were selected and randomly divided into treatment group and control group by the random number table method, with 38 cases in each group. The control group was taken routine therapy, the treatment group was taken acupuncture at Baliao cooperated with pelvic muscles exercise, continuous treatment for one month. The changes of symptom scores before and after treatment in the two groups were observed, and the clinical efficacy of the two groups was compared. Results The symptom scores of urinary urgency, whether urine can be tolerated before the toilet, enuresis nocturna interval, enuresis nocturna times, 24 h leakage of urine, induced factors of urination were all significantly lower than those before treatment （P 〈 0.05）, and the symp- tom scores above in the treatment group after treatment were all lower than those of control group, the differences were statistically significant （P 〈 0.05）. The total effective rate in the treatment group was 94.7%, which of the control group was 78.9%, the difference was statistically significant （P 〈 0.05）. Conclusion Acupuncture at Baliao cooperated with pelvic muscles exercise in the treatment of urinary incontinence after electro-vaporization of prostate has definite effects, and it is easy to operate, which is a good method to treat urinary incontinence after electro -vaporization of prostate, it is worthy of wide promotion and application.
