摘　　要：目的探讨加温湿化高流量鼻导管通气(HHFNC)治疗新生儿肺炎的效果。方法选择2015年12月~2016年11月在佛山市南海区第四人民医院新生儿科住院治疗的60例新生儿肺炎患儿,采用简单随机法分为两组,每组30例,均给予常规护理治疗,对照组采用经鼻持续气道正压通气(n CPAP)治疗,观察组采用HHFNC氧疗。观察两组治疗24 h后的动脉血气分析、临床症状体征变化以及并发症发生情况。结果治疗24 h后,两组患儿治疗前后PaO_2、PaCO_2比较,差异有统计学意义(P<0.05)。两组患儿治疗总有效率比较,差异无统计学意义(P>0.05);两组患儿使用镇静剂比例及并发症发生率比较,差异有统计学意义(P<0.05)。结论 HHFNC是治疗新生儿肺炎安全有效的呼吸支持模式。Objective To investigate the effect of heated humidified high flow nasal cannula （HHFNC） on the treatment of ,neonatal pneumonia.Methods 60 children with neonatal pneumonia hospitalized in Paediatrics of the Fourth People＇s Hospital of Nanhai District from December 2015 to November 2016 were selected,and they were randomly divided into two groups which had 30 cases in each group.Both two groups were given routine nursing treatment,children in the control group were given nasal continuous positive airway pressure （nCPAP） treatment,which in the observation group were given HHFNC therapy.and then,the results of arterial blood gas analysis,the changes of clinical symptoms and complications were observed in both two groups after 24 h treatment.Results After 24 h treatment,the differences of the results of PaO2,PaCO2 from blood gas analysis of the children in the both two groups between before and after treatment,the difference was statistically significant （P〈0.05）.The total effective rate of the observation group and the control group had no significantly differences （P〉0.05）;the use of sedatives and the incidence of complications had a significantly difference between the two groups （P〈0.05）.Conclusion HHFNC is a safe and effective respiratory support model for the treatment of neonatal pneumonia.
