阿利沙坦酯治疗老年轻-中度原发性高血压的有效性和安全性观察

摘　　要：目的评价阿利沙坦酯治疗老年轻-中度原发性高血压(高血压)的有效性和安全性。方法将60例轻-中度高血压老年患者随机分为阿利沙坦酯组和缬沙坦组,治疗前及治疗12周后监测诊室血压(CBP)、心率、心电图、血常规、尿常规、生化指标,治疗前与治疗12周结束后行24 h动态血压监测(ABPM)。结果治疗12周末,两组的诊室血压,24 h、白昼、夜间平均血压,血压负荷值及夜间下降率均显著下降,与治疗前比较,差异有统计学意义(P<0.05),心率无显著变化;阿利沙坦酯降低白昼及诊室收缩压(SBP)和降低24 h、白昼、诊室收缩压幅度优于缬沙坦,差异有统计学意义(P<0.05)。两组治疗总有效率、舒张压(DBP)平均降压幅度、血压达标率、谷峰比值(T/P)及平滑指数(SI)比较,差异无统计学意义(P>0.05)。两组患者不良反应轻微,均能自行缓解,但在引起转氨酶升高方面比较,阿利沙坦酯更安全。结论阿利沙坦酯治疗老年轻-中度高血压疗效是安全、有效的。Objectives To evaluate the efficacy and safety of allisartan in elderly patients with mild and moderate essential hypertension. Methods Totally 60 cases of mild and moderate essential hypertension elderly patients were randomly divided into allisartan group and valsartan group. Changes of clinic blood pressure （CBP） , heart rate, electrocardiogram, blood and urinary laboratory examination were observed before and after 12-week treatment. The 24- hour ambulatory blood pressure monitoring （ABPM） was investigated at the end of placebo and treatment period. Results After 12 weeks treatment, clinic blood pressure, 24 hours blood pressure, day and night average blood pressure, blood pressure load value at night and decrease rate of the two groups significantly decreased （P〈0.05） , but heart rate had no significant change （P〉0.05）. Allisartan reduced daytime and clinic systolic blood pressure （SBP） and reduced 24 h SBP, daytime SBP, clinic SBP amplitude better than valsartan did （P〈0.05）. There were no significant differences between the two groups in total effective rate, diastolic blood pressure （DBP） mean decreasing amplitude, blood pressure compliance rate, trough to peak （T/P） ratio and smoothness index （SI） （P〉0.05）. Mild adverse reactions of the patients in the two groups can alleviate by oneself, but concentration of transaminase was higher in valsartan group than in allisartan group （P〈0.05）. Conclusions Allisartan applied in elderly patients with mild to moderate essential hypertension is safe and effective.
