摘 要:目的介绍并评价一种对非标准体位头颅CT扫描患者进行颅内病灶手术定位的改进方法。方法选取近3年来218例脑内血肿手术患者,常规定位对照组74例,调整基线定位组144例,根据血肿与骨窗中心或穿刺点的位置关系,设计评价标准,根据定位准确性评分比较定位准确性。结果常规定位对照组定位准确性评分平均为4.30±1.13分。调整基线定位组定位准确性评分平均为4.70±0.63分。行Z检验,两组间比较Z=2.91>1.96,两组间有显著性差异。结论颅脑CT调整基线定位法准确度更高,有较高的临床应用价值。Objective To introduce and evaluate the modified method for precise localization of craniocerebral lesions showed in an offstandard orbitomeatal line computed tomography. Methods 218 patients with intracerebral hemorrhage accepted operation were reviewed,and were divided into two groups. The hemorrhages of 74 patients were localized in conventional method before the operation(conventional location group, CLG), while 144 patients were localized by modified method(modified location group,MLG). The accuracy was assessed either by the position of hemorrhage and bone window observed in the operation, or the ubiety and orientation among hemorrhage and puncture tunnel displayed in computed tomography imaging. A new standard for evaluation was established to estimate the accuracy of localization. Results The average score of location accuracy from CLG was 4.30+1.13 ,and the average score of location accuracy from MLG was 4.70±0.63.Z test was implied to compare the two groups. Z=2.91〉1.96,difference had statistical significance. Conclusion Compared to the conventional method, the modified method for precise localization of craniocerebral lesions had more accurate, it is worthy of clinic application.
