摘 要:目的:评价5-氨基酮戊酸光动力疗法治疗甲真菌病的临床效果。方法:将入选的40例患者随机分成治疗组和对照组,其中治疗组有病变趾甲56只,对照组有病变趾甲61只。以10%尿素软膏涂抹病变趾甲,并用塑料膜覆盖10 h,尽量刮除病甲表面的甲屑。以20%的ALA溶液涂抹于治疗组的病甲表面,用塑料膜覆盖,外层用铝箔包裹,避光5 h。最后用635 nm波长的激光照射病甲,1次/周,PDT能量密度100 J/(cm^2·次),总能量为600~700 J/cm^2。对照组的病变趾甲则用1%特比萘芬乳膏涂抹,并用塑料膜覆盖,2次/周,连续6~7周。于治疗结束9个月后进行疗效评价。结果:治疗组痊愈率为14.28%,总有效率为37.50%,真菌培养阴性率为32.14%,对照组的痊愈率、总有效率、真菌培养阴性率分别为14.75%、39.34%、37.70%,两组差异无统计学意义(P>0.05)。结论:5-氨基酮戊酸光动力疗法对甲真菌病有一定疗效,值得深入研究。Objective: To evaluate the effect of 5-aminolevulinic Acid photodynamic therapy on onyehomyeosis.Method: 40 patients were randomly divided into treatment group and control group, there are 56 lesions toenails in the treatment group and 61 lesions toenails in the control group.10% Urea Ointment was smeared on lesions toenails, and lesions toenails were covered with plastic film for 10 hours.Then scraped the nail shavings as possible.20% ALA solution was applied to the surface of lesions toenails in the treatment group, then lesions toenails were covered with plastic film.The outer layers were covered with aluminum foil, avoiding light for 5 hours.Finally lesions toenails were irradiated by 635 nm wavelength laser once a week, the energy density of the PDT was J/cm .1% Terbmafine smeared on lesions toenails in the control group twice a week for 6-7 weeks, 100 J/(cm^2 · time), the total energy was 600-700 J/cm^2 cream was and lesions toenails were covered with plastic film.The curative effect was evaluated after the treatment had been doned for 9 months.Result: The cure rate of the treatment group was 14.28%, and its effective rate was 37.50%, fungal culture positive rate was 32.14%.At the same time the cure rate of the control group was 14.75%, and its effective rate was 39.34%, fungal culture positive rate was 37.70%.The cure rate, efficiency rate and fungal culture positive rate had no significant difference between the treatment group and the control group(P〉0.05).Conclusion: 5-aminolevulinic Acid photodynamic therapy has certain curative effect on onychomycosis, and it is worthy of further study.
