“云端药历”在合理用药管理中的作用及取得的成效

摘　　要："云端药历"是台湾卫生福利部中央健康保健署利用云端技术于2013年开发的患者用药历史查询系统,该系统能对患者的用药记录进行归档,并存储于健保资讯网络服务系统。在医师处方药品和药师药事照护时,可通过"云端药历"查询患者最近3个月内的用药记录,从而避免发生患者重复用药和药物相互作用等情况,并有助于减少药品浪费。本文介绍"云端药历"在台湾合理用药管理中的作用及取得的成效。The patient-centered Pharmacloud system was developed in 2013 by the National Health Insurance Administration, Ministry of Health and Welfare in Taiwan and using cloud technology, which can continuously collect all the medication data of patients from each hospital, clinic and pharmacy and then upload to Virtual Private Network. The medication record of patients in the last 3 months can be checked by Pharmacloud system when physicians are going to prescribe or pharmacists are going to perform caregive so as not only to avoid the unsafe medication such as duplication medication, drug interaction and so on but also to help reduce drug waste. The roles and effects of Pharmacloud system in rational medication management in Taiwan are introduced in this article.
