改良PPH结合消痔灵注射治疗混合痔的临床疗效

摘　　要：为探讨改良PPH结合消痔灵注射治疗混合痔的临床疗效，回顾分析2015年5月至2016年4月于我院采用改良PPH结合消痔灵注射治疗的60例混合痔患者资料。结果显示，本组60例患者均顺利完成手术，手术时间为25～40min，平均30．0min。住院时间为5～7d，平均6d。切除痔及痔上黏膜环宽度1．0～2．5cm，平均1．5cm，深度达浅肌层，吻合口在齿状线上1．0～2．0cm，平均1．5cm。切除痔及痔上黏膜环均完整。术后尿潴留22例，肛门轻微疼痛36例，肛缘轻度水肿11例，首次排便闲难18例。随访6个月至1年，均无复发，无肛门失禁、肛门狭窄、直肠阴道瘘及直肠穿孔等并发症发生，肛门外观平整。结果表明，改良PPH结合消痔灵注射治疗混合痔疗效确切，术后疼痛轻，并发症少，值得临床推广应用。To explore the clinical effect of mixed hemorrhoids treated with modified PPH plus Xiaozhiling injection,authors retrospectively analyzed the data of 60 cases who admitted and adopted above combina- tion therapy for their mixed hemorrhoids in author＇s hospital from May 2015 to April 2016 yr..As results, the 60 patients were all performed surgery,operative time was 25 40 min（averaged 30.0 min）,hospitaliza- tion time was 5-7 days（averaged 6 days） ;the width of resected hemorrhoids and mucosa ring above hemor- rhoids was 1.0-2.5 cm（averaged 1.5 cm）,resected depth was to superficial myo-layer,stoma was at 1.0-2.0 cm above dental line（averaged 1.5 cm）;the resected hemorrhoids and mucosa ring over them were all in- tact;after operation 22 cases suffered from uroschesis,36 from mild pain of anus,11 from mild ano-edge e- dema, 18 from dyschesia at the first time of defecation;the follow up of 6-12 months did no find any recur- rence, copracrasia, anal stenosis, rectovaginal fistula and rectal perforation, etc complications, what＇s more, the appearance of anus was neat and smooth.Results show that above modified PPH plus Xiaozhiling injec- tion in the treatment of mixed hemorrhoids has certain effect, mild postoperative pain and less complication,worthy to be popularized and used clinically.
