针刺联合生物反馈治疗盆底失弛缓综合征的临床研究

摘　　要：为探讨针刺联合生物反馈治疗盆底失弛缓综合征的临床疗效，将120例盆底失弛缓综合征患者随机分为针刺组、生物反馈组、针刺＋生物反馈组，每组40例。针刺组针刺中鼹、下髂、天枢、气海、脾俞、大肠俞、足三里、百会和大椎，生物反馈组行盆底生物反馈治疗，针刺＋生物反馈组同时予以针刺和生物反馈治疗。对比3组治疗效果及治疗前后肛管直肠测压结果和盆底肌电图检测结果。结果显示，针刺＋生物反馈组总有效率（92．5％，37／40）明显高于针刺组（82．5％，33／40），亦明显高于生物反馈组（87．5％，35／40），P〈0.01或P〈0．05。3组治疗后力排肛管压均较治疗前降低，P〈0．01；生物反馈组、针刺＋生物反馈组治疗后直肠静息压、力排直肠压较治疗前均降低，P〈0．05或P〈0．01；针刺＋生物反馈组治疗后肛管静息压较治疗前降低，P〈0．05；针刺＋生物反馈组治疗后肛管静息压、力排肛管压、力排直肠压明显低于针刺组治疗后，P〈0．05。3组治疗后盆底肌电图前基线、后基线较治疗前均显著降低，P〈0．01；针刺＋生物反馈组治疗后盆底肌电图变异性亦较治疗前降低，P〈0．01。结果表明，针刺、生物反馈、针刺联合生物反馈治疗盆底失弛缓综合征均可取得一定疗效，其中针刺联合生物反馈疗较更佳。To explore the clinical curative effect of acupuncture combined biofeedback for the treatment of achalasia pelvic floor syndrome（APFS）,120 cases of APFS patients were randomly divided into acupunc- ture group, biofeedback group, acupuncture plus biofeedback group, 40 cases for＇each. In acupuncture group, Zhongliao （BL33 ）, Xialiao （BL34 ）, Tianshu （ ST25 ）, Qihai （ RN6 ）, Pishu （ BL20）, Dachangshu （BL25 ）, Zusanli（ST36） ,Baihui（BU20） and Dazhui（DU14） points were needled; in the biofeedback group,pelvic floor biofeedback therapy was adopted; in acupuncture plus biofeedback group, acupuncture and biofeed- back treatment were all adopted at the same time.The curative effect,anal canal and rectal pressure results and tested results of pelvic floor electromyography before and after the treatment were compared among the three groups.As results,total effective rate （92.5% ,37/40） in acupuncture plus biofeedback group was significantly higher than that in acupuncture group （82.5% ,33/40）,and also significantly higher than the biofeedback group （87.5%,35/40, P 〈 0.01 or P 〈0.05）.The force of anal canal pressure of three groups after treatment was reduced compared with that before treatment（ P 〈 0.01）; Rectal rest pressure, force of rectal pressure of the biofeedback group, acupuncture plus biofeedback group after treatment was all re- duced compared with that before treatment （P 〈0.05 or P 〈0.01）; Anal rest pressure of acupuncture plus biofeedback treatment group after treatment was reduced compared with that before treatment（ P 〈 0.05） ;anal canal rest pressure,force of anal canal pressure,force of rectal pressure of acupuncture plus bio- feedback group after treatment was obviously lower than that of acupuncture group after treatment（ P 〈 0.05）.The front and post baseline of pelvic floor electromyogram in the three groups after treatment were significantly lower than that before treatment（ P 〈0.01） ; in the variability of pelvic fl
