摘  要:目的探讨实时组织弹性成像技术(RTE)对猪下肢枪弹伤伤道周围失活组织区域的诊断价值。方法制作猪下肢软组织枪弹伤动物模型,分别于致伤前和致伤后不同时相点,应用RTE技术,动态观察枪击后致伤区域,并与病理检查结果进行对比分析。结果致伤后即刻,原发伤道内RTE图像呈现"软"的特征,其范围由原发伤道内积血多少决定;随着时间的推移,致伤后24 h,原发伤道内血凝块机化,呈现"硬"的特征。原发伤道及周围组织致伤后即刻,RTE由内向外呈现"软-硬-软"或"软-硬"的变化特征。RTE变硬区域,病理检查显示为失活组织,其外相对较"软"区域病理显示为正常组织。结论 RTE技术可以快速分辨原发伤道周围失活组织区域,可为肢体枪弹伤失活区的准确判断提供重要的影像学依据。Objective To explore the value of real-time tissue elastography (RTE) in the diagnosis of the devitalized tissue area around the gunshot wound of lower limb in pigs. MeAods Animal models of soft tissue gunshot wounds of lower limb in pigs were prepared. RTE was applied at different time points before and after the wounding to observe the wounded area in a dynamic manner and compare with the result of pathological examination. Results RTE image showed “soft” features in the primary wound tractimmediately after the wounding, with the scope determined by the amount of hematocele in the primary wound tract. As time went on, 24 h after the wounding, the blood clot in the primary wound tract showed organization and “hard” features. The RTE of the primary wound tract and peripheral tissues immediately after the wounding showed inside-out variation features of "soft-hard-soft" or “soft-hard”. According to pathological examination, the hardened areas in RTE were devitalized tissues, while the outside relatively “soft” areas were normal tissues. Conclusion RTE can quickly identify the devitalized tissue areas around the primary wound tract and this provides important imaging basis for accurate diagnosis of the devitalized area of gunshot wounds in limbs.
