腋下副乳腺3种不同治疗方法的疗效及愈后美观度比较

摘　　要：目的探讨3种不同术式治疗腋下副乳腺的疗效及愈后美观度。方法选取腋下副乳腺患者90例(副乳腺138个),随机分为A、B、C 3组,各30例,分别实施传统副乳腺切除术、腋部皮纹小切口潜行切除术、肿胀脂肪抽吸配合乳腔镜切除术治疗。比较3组手术基本情况和治疗效果,并评估愈后美观满意度。结果与A、B组相比,C组单侧平均手术时间明显较长,但单侧平均失血量显著较少,住院时间显著较短,且未出现局部明显疼痛和上肢活动受限(P<0.05);与A组相比,B组单侧失血量明显较少,住院时间明显较短(P<0.05),而单侧手术时间、局部明显疼痛和上肢活动受限发生情况比较无统计学差异(P>0.05)。C组愈后美观满意度为93.33%,显著高于A、B组的66.67%、73.33%(P<0.05),A、B两组愈后美观满意度比较无统计学意义(P>0.05)。结论肿胀脂肪抽吸配合乳腔镜切除术治疗腋下副乳腺,可取的更为显著的临床疗效和更高的愈后美观满意度,且手术安全性更高。Objective To explore the effects and aesthetic satisfaction of three different therapies for armpit mamma accessoria.Methods A total of 90 patients with armpit mamma accessoria(the number of mamma accessoria: 138) were selected and randomly divided into groups A, B and C(n=30, respectively). The three groups were treated with traditional resection of mamma accessoria,small incision resection by stealth at armpit and tumescent liposuction combined with endoscopic resection, respectively. The basic conditions and effects of operation among the three groups were compared, and the aesthetic satisfaction after healing was estimated.Results The unilateral mean operation time in group C was significantly longer than that in groups A and B, but the unilateral mean blood loss in group C was significantly less and the LOS much shorter, and there was no local pain and upper limb activity limitation(P < 0.05); The unilateral mean blood loss in group B was much less than that in group A and the LOS much shorter(P < 0.05), but there was no statistical difference in the unilateral operation time, local pain and upper limb activity limitation between the two groups(P > 0.05). The aesthetic satisfaction after healing in group C was 93.33%, which was significantly higher than that in groups A and B(66.67% and 73.33% respectively)(P < 0.05). The aesthetic satisfaction after healing in groups A and B showed no statistical difference(P > 0.05). Conclusion The treatment of armpit mamma accessoria with tumescent liposuction combined with endoscopic resection can realize more significant effects, higher aesthetic satisfaction after healing and higher operation safety.
