隐匿性下肢动脉损伤25例的介入治疗

摘　　要：目的探讨隐匿性下肢动脉损伤的腔内治疗方法。方法收集25例外伤合并下肢血管损伤病人进行回顾性分析。结果25例动脉损伤行介入手术病人术后患肢血供正常,未见间歇性跛行及其他缺血症状;4例髂动脉覆膜支架病人,未出现动脉支架内再狭窄,1例髂静脉支架发生血栓,予以静脉导管溶栓后好转,未出现慢性静脉功能不全(PTS)表现。结论隐匿性下肢动脉损伤因损伤不易发现及手术处理困难,适合行腔内介入手术,其优点是损伤小,能够快速确定损伤位置,缩短手术时间,减少出血,达到及时救治病人的目的。Objective To discuss the endouvascular treatment of concealed lower extremity arterial injury. Methods Twenty-five trau-ma patients with lower extremity arterial injury in our hospital from January 2013 to January 2015 were collected with retrospective anal-ysis. Results The blood supply of affected limbs of 25 cases is normal after endouvascular operations. Four patients did not appear stent restenosis who were treated with covered stent,and one case had thrombus after being treated with iliac vein stent. Then this pa-tient was cured by using the method of catheter thrombolysis without appearing chronic venous insufficiency of lower limb. Conclusions Concealed lower extremity arterial injury not be discovered easily,and it is difficult to treat by operation. So it is suitable to endouvas-cular operation. Endouvascular operation has many advantages, such as it can reduce damage, can confirm damage location quickly, shorten operation time, reduce the amount of bleeding and can get the goal of treating patients timely.
