摘 要:目的探讨糖尿病患者血清胰岛素样生长因子-1(IGF-1)、肿瘤坏死因子(TNF-α)水平与颈动脉粥样硬化斑块的相关性。方法应用超声检测233例患者双侧颈动脉内膜-中膜厚度,根据超声检查结果将其分为:病例组颈动脉粥样硬化斑块阳性患者120例和对照组颈动脉粥样硬化斑块阴性患者113例,并釆用免疫放射分析法检测两组患者的血清IGF-1和TNF-α水平。结果血清IGF-1水平病例组与对照组比较降低明显,差异有显著统计学意义(P<0.01);患者血清IGF-1水平与颈动脉粥样斑块的程度呈显著负相关(P<0.01)。血清TNF-α水平在颈动脉粥样硬化斑块阳性患者与对照组比较升高显著,差异有显著统计学意义(P<0.01);患者血清TNF-α水平与颈动脉粥样斑块的程度呈显著正相关(P<0.01)。结论 IGF-1、TNF-α参与糖尿病动脉粥样硬化患者大血管病变的发生、发展,并与颈动脉粥样斑块程度密切相关。Objective To investigate the correlation among serum IGF-1,TNF-α and carotid atherosclerosis plaque in patients with diabetes.Methods The bilateral carotid intima-media thickness (IMT) of 233 patients were measured with ultrasound.According to the result of ultrasound,the patients were divided into case group with carotid atherosclerosis plaque (n=120) and control group without the plague (n=113),and the levels of serum IGF-1and TNF-α of the patients were determined with immunoradiometric assay.Results Compared with the control group,the case group had notably lowered IGF-1 level,with statistically significant difference (P〈0.01).There was a negative correlation between IGF-1 level and the degree of carotid atherosclerosis plaque (P〈0.01).The TNF-α level increased obviously in the case group in comparison with the other group,with statistically significant difference (P〈0.01),presenting a positive correlation between TNF-α level the degree of carotid atherosclerosis plaque (P〈0.01).Conclusion IGF-1 and TNF-α are involved in the occurrence and development of macroangiopathy in diabetes patients with atherosclerosis,and closely related to the degree of carotid atherosclerosis plaque.
