腹腔镜下不同手术方式对输卵管妊娠卵巢功能的影响

摘　　要：目的探讨腹腔镜下不同手术方式对输卵管妊娠卵巢功能的影响。方法随机将我院120例行输卵管异位妊娠手术的输卵管妊娠患者分为甲、乙两组,每组各有患者60例,甲组实施腹腔镜输卵管切除术,乙组实施腹腔镜输卵管开窗妊娠物取出术。结果术后甲组患者的FSH、E2以及LH水平要显著高于乙组,P水平要显著低于乙组(P<0.05);乙组患者术后的卵巢截面积、窦状卵泡数、PSV以及EDV水平要显著高于甲组(P<0.05)。结论腹腔镜下输卵管切除术治疗输卵管妊娠,患者的FSH、E2以及LH水平会显著上升,卵巢储备功能受到一定影响,在患者情况允许的条件下,应尽可能实施保守性手术治疗,尽量保护输卵管储备功能。Objective To investigate the effect of different operation methods on ovarian function of tubal pregnancy.Methods 120 cases of pregnancy patients with ectopic pregnancy fallopian tube were randomly divided into two groups,group A and group B,in our hospital,with 60 patients in each group.Group A received the laparoscopic tubal resection operations,group B adopted the laparoscopic tubal fenestration and embryo removal operations.Results The level of FSH,E2 and LH of the patients in group A after operation was significantly higher than that in group B,while the level of P was significantly lower than that in group B （P〈0.05）.The patients in group B,whose postoperative ovarian cross-sectional area,the number of antral follicles,PSV and EDV levels were significantly higher than in group A （P〈0.05）.Conclusion Laparoscopic salpingectomy for tubal pregnancy patients will bring about significant increase of patients＇ FSH,E2 and LH levels,and ovarian reserve function will be affected.With permission from the patients,conservative surgery should be implemented to protect the reservation function of fallopian tube.
