早中孕期胎儿畸形超声筛查方法分析

摘　　要：目的探讨二维超声和二维联合实时三维超声在孕早中期筛查胎儿畸形的价值,为临床推广无创伤筛查方法提供基础资料依据。方法回顾性分析中南大学湘雅医学院附属海口医院于2013年1月—2015年6月共4 929例产妇经产前二维超声和二维联合实时三维超声对比检查结果,并对比分析追踪引产或分娩的结果。结果引产或分娩后证实胎儿畸形151例,二维超声检出的胎儿畸形108例,准确率为71.5%,二维超声联合实时三维超声检出胎儿畸形135例,准确率为89.4%,差异有统计学意义(P<0.05)。二维超声联合实时三维超声在胎儿神经系统畸形、先天性心脏畸形、消化道畸形、唇腭裂、两种及以上多种畸形等的检出准确率要明显高于单独使用二维超声,差异有统计学意义(P<0.05)。结论二维超声联合实时三维超声检查可以明显提高胎儿畸形的筛查率,有助于优生优育。Objective To analyze and explore the value of two-dimensional ultrasound and two-dimensional ultrasoundcombined with three-dimensional ultrasound in early and middle pregnancy fetal malformation screening, we provide basicdata for promoting non-invasive screening method in clinic. Methods To retrospectively analyze the result of obstetrical 4929 cases by using two-dimensional ultrasound and two-dimensional ultrasound combined with three-dimensional ultrasound,to contrast,we analyzed and followed the result of induced abortion or delivery. Results Among the 4 929 obstetrical cases,the fetal malformations were confirmed 151 cases, 108 fetal malformations were found by single two-dimensional ultrasound,but after induced abortion and delivery, the diagnostic accuracy was 71.5%,while 135 fetal malformations were found bycombined detection, the diagnostic accuracy was 89.4%. The difference was statistically significant（P〈0.05）. The diagnosticaccuracy of the fetal malformation, such as neural malformation, congenital heart malformation, digestive tract malformation,cleft lip and palate, by using combined detection was significantly higher than single two-dimensional ultrasound. Thedifference was statistically significant（P〈0.05）. Conclusion Two-dimensional ultrasound combined with three-dimensionalultrasound can markedly improve the fetal malformation screening rate, and they can contribute to prenatal and postnatal care.
