摘  要:目的评价应用国产160 W绿激光施行功率选择性汽化术治疗大体积前列腺增生的疗效和安全性。方法 2013年5月至2015年3月,苏州大学附属第二医院应用国产160 W高功率绿激光系统,施行经尿道前列腺汽化术(PVP)治疗前列腺增生(BPH)。本组BPH患者37例,平均年龄71(59~92)岁,前列腺体积平均116(81~273)ml,国际前列腺症状评分(IPSS)评分平均27(15~33)分,最大尿流率(Qmax)平均6.0(3.6~9.3)ml/s,膀胱残余尿(RUV)平均252(18~1432)ml。比较患者手术前后血红蛋白变化、记录手术时间、术后留置导尿管时间、术后住院时间以及手术后并发症的情况。术后3个月随访IPSS、生活质量评分(QOL)、Qmax的变化。结果本组手术时间平均96(35~170)min,激光耗能平均520(345~1082)k J,所有手术均一次完成,术后留置导尿管1~6 d,平均3 d。32例患者术后完成3个月周期的随访,IPSS由术前27分下降至术后3个月8(3~18)分(P<0.01);Qmax由术前6(4~9)ml/s上升至术后3个月15(13~18)ml/s(P<0.01);QOL由术前4(3~6)分下降至术后3个月2(1~4)分(P<0.01)。结论应用国产160 W绿激光施行PVP治疗大体积BPH同样安全、有效、恢复快,是目前治疗大体积BPH理想的微创治疗方法。Obiective To evaluate the safety and efficacy of domestic 160-Watt high power greenlight photoselective vaporization for large volume prostate in patients with benign prostate hyperplasia (BPH). Methods From May 2013 to March 2015, 37 cases with BPH treated with 160-Watt high power green laser were enrolled. The mean age was 71 years (range from 59 to 92), the volume of prostate in all the patients was over 80 ml. The volume of the prostate, IPSS, Qmax and QOL were recorded before and 3 months after the operation. Hemoglobin changes pre- and postoperation, catheter time and hospitalization time were recorded and compared. Results The average operation time was 96 min (35-170 min), the average consumption of energy was 520 kJ (345-1082 kJ). The catheter was indwelled for 1 to 6 days (average 3 days). Thirty-two patients were followed up 3 months. Mean Qmax increased from 6 ml/s preoperatively to 15 ml/s. IPSS and QOL improved from 27, 4 to 8, 2 respectively. All the differences were statistically significant (P〈0.01). Conclusion The domestic 160-Watt greenlight high power laser is an effective and safe method for the treatment of BPH, and especially appropriate to patients with large volume prostate.
