宫颈电热圈环切术后宫颈粘连的相关因素分析

摘　　要：目的分析宫颈电热圈环切术后患者发生宫颈粘连的影响因素。方法回顾性分析我院442例于2008年1月至2016年6月收治并行宫颈电热圈环切术治疗的高级别宫颈上皮内瘤变患者的临床资料。结果本院2008年1月至2016年6月,宫颈电热圈环切术后宫颈粘连的发生率为3.85%。单因素分析发现>45岁女性中宫颈粘连发生率为8.25%,高于≤45岁女性宫颈粘连发生率为2.61%,差异有统计学意义(P<0.05)。术后创面感染患者宫颈粘连的发生率为25%,高于未感染患者宫颈粘连的发生率3.65%,差异有统计学意义(P<0.05)。在多因素分析中,年龄、感染是导致该并发症发生的高危因素,危险度分别为0.267及13.875。结论随访过程中及时发现、尽早治疗细菌感染,对于年龄>45岁患者术后加强随访,及时、早期发现宫颈粘连。Objective Analysis of the influencing factors of LEEP patients after cervical adhesions. Methods Retrospective analysis of clinical data of 442 cases in January 2008 – June 2016 year from the parallel LEEP treatment of high grade cervical intraepithelial neoplasia. Results The January 2008-June 2016 year period, LEEP cervical adhesion rate was 3.85%. The single factor analysis found that more than 45 year old woman in cervical adhesions occurred at a rate of 8.25%, higher than that of less than 45 year old female cervical adhesion rate was 2.61%, the difference was statistically significant（P〈0.05）. The incidence of cervical adhesions in patients with postoperative wound infection was 25%, which was higher than that of non infected patients. The difference was statistically significant（P〈0.05）. In multivariate analysis, age and infection were the risk factors for the occurrence of complications, the risk was 0.267 and 13.875, respectively. Conclusion In the course of follow-up, timely detection and treatment of bacterial infection as soon as possible, for more than 45 years old patients with postoperative follow-up, timely, early detection of cervical adhesions.
