化瘀解毒饮联合窄谱中波紫外线治疗斑块型银屑病临床研究

摘　　要：目的:观察化瘀解毒饮联合窄谱中波紫外线(NB-UVB)治疗斑块型银屑病临床疗效,初步探讨综合疗法对银屑病患者外周血淋巴细胞亚型的整体调节作用。方法:将患者随机分为两组,观察组39例采用化瘀解毒饮联合NB-UVB治疗,对照组36例单独用光疗仪NB-UVB治疗。结果:观察组银屑病面积和严重程度指数(PASI)评分下降较对照组明显(P<0.01),观察组总有效率94.9%,对照组总有效率77.8%,两组比较差异有统计学意义(P<0.05);治疗后两组患者生活质量指数(dermatology life quality index,DLQI)评分均有不同程度的改善,观察组显著优于对照组(P<0.01);较对照组,观察组外周血CD4^+淋巴细胞以及CD4^+/CD8^+明显升高(P<0.05),CD8^+明显降低(P<0.05)。结论:化瘀解毒饮联合NB-UVB治疗斑块型银屑病疗效显著,能够明显改善患者生活质量,延长复发间期,调节免疫平衡水平。Objective：To observe the therapeutic effect of Huayu Jiedu Drink combined with narrow band ul- traviolet in treating plaque psoriasis,and to analyze its regulating effect on lymphocyte subset in peripheral blood. Methods： Patients were divided into two groups randomly. The observation group（39 cases） was treat- ed with narrow band ultraviolet and Huayu Jiedu Drink,and the control group（36 cases） was treated with narrow band ultraviolet alone. Results：Compared with the control group,the psoriasis area and psoriasis severity index（PASI） score of the treatment group were significantly reduced（P〈0.01）. The effective rate in the treatment group was 94.9%,while it was 77.8% in the control group,with statistical difference（P〈 0.05）. The dermatology life quality index score was increased in both groups,but in the treatment group it was better than that in the control group（P〈0.01）. Compared with the control group,the number of CD4＋ T lymphocyte and and the ratio of CD4＋/CD8＋ significantly decreased（P〈0.05）,while the number of CD8＋ T lymphocyte significantly decreased（P〈0.05） in the treatment group. Conclusion： Huayu Jiedu Drink corn- bined with narrow band ultraviolet could treat plaque prolong the period between recurrence,and regulate the immune level.
