摘　　要：目的探讨主动脉瓣及二尖瓣机械瓣联合置换术后人工心脏瓣膜患者不匹配(VP-PM)现象的发生率及与术前患者情况及预后的关系。方法取2009年1月—2016年12月医院收治的行双瓣机械瓣置换患者70例,随机数字法分为对照组(n=35)和观察组(n=35)。对照组采用主动脉瓣治疗,观察组采用主动脉瓣及二尖瓣机械瓣联合置换术后人工心脏瓣膜治疗,比较2组术后人工心脏瓣膜患者不匹配(VP-PM)现象的发生率及与术前患者情况及预后的关系。结果观察组中6例术后发生VP-PM,发生率为17.14%,对照组9例发生VP-PM,发生率为25.71%,2组VP-PM发生率比较差异无统计学意义(P<0.05),2组VP-PM分级比较差异无统计学意义(P>0.05);VP-PM分级与患者肾衰竭及死亡率呈正相关性(P<0.05);VP-PM分级分级越高,肾衰竭和死亡率越高。结论双瓣机械瓣置换患者采用主动脉瓣及二尖瓣机械瓣联合置换术治疗后VP-PM发生率较高,且多发生在体重及体表面积较大者中,应采取措施干预处理。Objective To investigate the incidence of mismatch （VP-PM） in patients with prosthetic valve replacement after aortic valve and mitral valve replacement, and to evaluate the relationship between preoperative and postoperative patients. Methods a total of 70 patients who underwent mechanical valve replacement in our hospital from December 2014 to October -2016 were randomly divided into control group （n= 35 ） and observation group （n = 35 ）. The control group using aortic valve treatment, the observation group with aortic valve and mitral valve joint after replacement of artificial heart valve therapy, compared 2 groups of patients after arti- ficial heart valve （VP-PM） does not match the relationship between the incidence and the phenomenon of preoperative patients and prognosis. Results the incidence of VP-PM in the observation group 6 cases, the incidence rate was 17. 14%, control group 9 cases of VP-PM, the incidence rate was 25.71%, the prevalence of VP-PM in 2 groups had no significant difference （ P〈0.05 ）, the 2 groups had no statistically significant difference （P〉0. 05） VP-PM classification; VP-PM classification and patients with renal failure and the mortality rate was positively correlated （P〈0. 05 ） ; VP-PM grading is high, renal failure and mortality is high. Conclusion double valve valve replacement in patients with aortic valve and mitral valve replacement combined with a higher incidence of VP-PM after treatment, and occurs in weight and body area, measures should be taken to intervene.
