摘　　要：目的了解斑秃患者外周血中Th17和Treg细胞平衡状况,明确斑秃患者Th17和Treg细胞平衡状况是否受伴随的自身免疫性疾病影响。方法流式细胞术检测斑秃组患者和正常对照组外周血中Th17和Treg细胞在CD4^+T淋巴细胞中的比率。结果与正常对照组相比,斑秃组患者外周血Th17细胞比例明显升高,且在重型及伴有自身免疫性疾病斑秃组患者中最高,Treg细胞比例明显降低,在重型及伴有自身免疫性疾病斑秃组患者中最低。斑秃组患者外周血Th17/Treg细胞比值较正常对照组明显升高。结论斑秃患者外周血Th17/Treg细胞免疫失衡,且以重型及伴有自身免疫性疾病的斑秃患者更为显著。自身免疫性疾病可能加剧斑秃患者Th17/Treg细胞免疫失衡,从而导致斑秃进展。Objective To determine the status of Th17/Treg balance in peripheral blood of patients with alopecia areata（AA）and whether it is affected by co-existent autoimmune diseases.Methods The proportion of Th17 and Treg cells in CD4＋ T cells was analyzed with flow cytometry in AA and healthy controls.Results In comparison with healthy controls, the frequencies of Th17 cells were significantly higher in AA patients, with the highest in severe AA or AA accompanied by autoimmune diseases.In contrast, AA patients exhibited low frequencies of Treg cells, particularly in severe AA or AA with autoimmune diseases.The ratio of Th17/Treg was notably higher in patients with AA than in the controls.Conclusion Th17/Treg imbalance exists in AA patients, especially in severe AA or AA patients with other autoimmune diseases.Autoimmune diseases may worsen the imbalance of Th17/Treg, leading to progression of AA.
