摘  要:目的分析梅毒血清固定患者外周血CD4^+CD25^+Foxp3^+调节性T细胞(Treg细胞)数量及其功能的抑制情况,探讨细胞免疫功能与梅毒血清固定的关系。方法收集梅毒经规范治疗后出现血清固定患者30例及血清转阴对照患者30例的外周抗凝静脉血,应用流式细胞术分析两组患者外周血Treg细胞百分率,应用real-time PCR检测两组外周血单个核细胞Foxp3基因的表达情况,在自体APC存在的情况下检测两组Treg细胞体外抑制功能。结果梅毒血清固定组患者外周血Treg细胞占CD4^+T细胞比例(中位数:4.56%,25%~75%:3.34%~5.90%)明显高于转阴对照组(中位数:2.89%,25%~75%:2.05%~3.67%),两组相比差异有统计学意义(U=115,P=0.002<0.05);且Treg内转录因子Foxp3的表达量(17.03±2.51)也明显高于转阴对照组(7.65±0.68),两组Foxp3基因相对含量差异有统计学意义(t=8.821,P=0.043<0.05);固定组Treg细胞抑制百分率(中位数:82.35%,25%~75%:65.87%~96.60%)与对照组Treg细胞抑制百分率(中位数:85.95%,25%~75%:72.85%~96.35%)的差异无统计学意义(U=203,P=1.00>0.05)。结论梅毒血清固定者外周血Treg细胞数量增高引起的机体细胞免疫抑制可能是造成血清固定的重要原因之一。Objective To analyze the frequency and function of CD4+CD25+Foxp3+ regulatory T cells(Treg cells)in peripheral blood of patients with syphilis serofast in order to elucidate the relationship between cellular immune function and syphilis serofast.Methods After standard treatments, peripheral blood mononuclear cells(PBMCs)were prepared from patients with syphilis serofast and serologic negative.The flow cytometry was used to analyze the percentage of Treg cells in peripheral blood.Real-time PCR was used to assess expression levels of Foxp3 in PBMCs.The inhibitory function of Treg cells was determined in vitro in the presence of autologous APC.Results The proportion of the Treg cells in CD4+T cells were significantly higher in serofast vs.negative group(Serofast: median: 4.56%, 25th-75th percentile: 3.34%-5.90%; Negative: median: 2.89%, 25th-75th percentile: 2.05%-3.67%, U=115, P〈0.05).Relative expression levels of Foxp3 in Treg cells were also significantly higher in syphilis serofast than in negative group(17.03±2.51 vs.7.65±0.68, t=8.821,P〈0.05).The percentage inhibition of Treg cell function was similar between syphilis serofast and negative group(Serofast: median:82.35%,25th-75th percentile:65.87%-96.60%; Negative: median:85.95%,25th-75th percentile:72.85%-96.35%; U=203,P〉0.05).Conclusion Our findings suggest that increased Treg cells in peripheral blood could be one factor contributing to the development of syphilis serofast.
