摘　　要：目的观察分析乌司他丁治疗对脓毒症患者认知功能的影响。方法选择2014年1月~2015年12月于我院就诊的脓毒症患者108例,采用随机数字表法,分为观察组(n=54)与对照组(n=54),对照组患者给予常规对症治疗,观察组患者在此基础上给予乌司他丁静脉滴定,对比两组患者的治疗效果。结果治疗前,两组患者的MMSE评分、APACHEⅡ评分、SOFA评分、血清降钙素原、血清C-反应蛋白比较,差异无统计学意义(P>0.05);治疗后,观察组患者的MMSE评分显著高于对照组(P<0.05),APACHEⅡ评分、SOFA评分、血清降钙素原、血清C-反应蛋白均显著低于对照组(P<0.05);治疗前后两组患者的IgG、IgA、IgM、C3、C4比较,差异无统计学意义(P>0.05)。结论乌司他丁治疗能够改善脓毒症患者的认知功能,抑制机体炎性反应,但不影响脓毒症患者的非特异性免疫能力。Objective To observe and analyze the effect of ulinastatin on cognitive function in patients with sepsis. Methods 108 patients with sepsis treated in our hospital from January 2014 to December 2015 were divided into observation group（n=54） and control group （n=54）, according a random number table method. The control group was given routine symptomatic treatment, and the observation group treated with ulinastatin intravenous titration on the basis of routine symptomatic treatment. The therapeutic effect of patients in the two groups was compared. Results The differences in MMSE score, APACHE Ⅱ score, SOFA score, serum procalcitonin and serum C-reactive protein were not significant between the two groups before treatment （P〉0.05）. After treatment, the MMSE score of patients in the observation group was significantly higher than that in the control group （P〈0.05）. The APACHE Ⅱ score, SOFA score, serum procalcitonin and serum C-reactive protein were significantly lower than those in the control group（P〈0.05）. There were no significant differences in IgG, IgA, IgM, C3 and C4 between the two groups before and after treatment（P〉0.05）. Conclusion Ulinastatin treatment can improve the cognitive function of sepsis patients and inhibit the body＇s inflammatory response, but it does not affect the nonspecific immunity of sepsis patients.
