摘　　要：目的:探讨320排CT在胸部健康检查中实行个性化低剂量扫描的临床应用价值。方法:选择2014年6月-2016年12月来本院进行健康体检的人群242例,按体质指数标准分成四组,使用东芝公司320排螺旋CT进行常规剂量及各级低剂量扫描并进行后处理,通过分析软件记录扫描时间(T)、CT剂量指数(CTDI)、剂量长度乘积(DLP),测定图像CT值标准差(SD),并对所有图像进行三维重建(3D)、最小密度投影(MIP)、多平面重建(MRP)等处理,分析比较常规剂量及各级低剂量扫描的辐射剂量减低差异及图像的质量差别。结果:CT扫描图像的CT值标准差(SD)随着剂量的降低而增加,且在降到15 m A时升高突然明显。第二组至第五组的CT值标准差(SD)与第一组比较,差异均有统计学意义(P<0.05)。不同体指数的人群,通过降低各级剂量,分析其图像质量,得出了各组在最佳低剂量扫描条件下,保证图像满足诊断要求的扫描方法。结论:320排螺旋CT在不同体质量指数中实行个性化低剂量扫描可用于大规模胸部体检。Objective： To explore the clinical application value of 320 slice spiral CT of the chest health examination in the implementation of personalized low dosage scanning.Method： 242 people who took physical examination in our hospital from June 2014 to December 2016 were selected, they were divided into four groupsaccording to body mass index.320 slice spiral CT of Toshiba were adopted for the scanning of conventional dosage and low dosage at different levels with post-processing.Through the analysis of scanning time software （ T ）, CT dose index （ CTDI ）, dose length product （ DLP ）, determination of image CT value standard deviation （ SD ）, and 3D reconstruction of all images （ 3D ）, minimum intensity projection （ MIP ）, multi planar reconstruction （ MRP ） etc, the difference of radiation dose reduction and image quality between conventional dose and low- dose scanning were compared.Result： The CT standard deviation （ SD ） of the CT scan images increased with the decrease of the dose, and increased significantly at 15 mA.The CT standard deviation （ SD ） of the second group to the fifth group were compared with the first group, the differences were statistically significant （ P〈0.05 ） . Population with different body mass index, by reducing the dose at all levels and analyzing the image quality, the scanning method of each group to ensure that the image meets the diagnostic requirements under the condition of the best low dose scanning.Conclusion： 320 slice spiral CT in the low body mass index in the implementation of personalized low-dose scanning can be used for large-scale chest examination.
