摘 要:目的通过对慢阻肺患者外周血淋巴细胞NLRP3 mRNA及血清中炎症介质IL-1β、IL-18的检测,探讨NLRP3炎症小体在慢阻肺发病中的作用。方法选取60例确诊AE慢阻肺患者,根据有无呼吸衰竭,分为2组,每组30例,急性加重期患者经治疗症状体征好转达到缓解期水平后作为慢阻肺组,同期门诊健康体检者30例作为健康对照组。采用ELISA检测各个组血清中IL-1β、IL-18的浓度,采用PCR检测各组中NLRP3 mRNA的表达量。结果 AECOPD组和慢阻肺缓解组患者的NLRP3 mRNA表达量和血清中IL-1β和IL-18的浓度均显著高于健康对照组(P<0.05);AE慢阻肺组患者的NLRP3 mRNA表达量和血清中IL-1β和IL-18的浓度均显著高于慢阻肺缓解组(P<0.05);慢阻肺合并呼吸衰竭患者的NLRP3 mRNA表达量和血清中IL-1β和IL-18的浓度显著高于未合并呼吸衰竭患者(P<0.05);慢阻肺患者NLRP3 mRNA表达量与血清中IL-1β、IL-18的浓度呈正相关,相关系数r分别为0.69和0.70(P<0.05);IL-1β的浓度与IL-18的浓度呈正相关,相关系数r=0.65(P<0.05)。结论 NLRP3和IL-1β、IL-18参与慢阻肺的炎症反应,其浓度升高导致慢阻肺病情严重程度增加。Objective To explore the role of NLRP3 inflammasome in the occurrence and development of COPD through the detection of NLRP3 mRNA in peripheral blood lymphocyte and the concentration of IL-1β and IL-18 in plasma of COPD patients. Methods 60 patients with AECOPD were selected and divided into 2 groups according to whether there was no respiratory failure, 30 cases in each group. Patients with acute exacerbation of symptoms and signs after improvement to achieve remission level were taken as the COPD group, and 30 cases of healthy people at the same period were selected as the control group. The concentration of IL-1β and IL-18 was measured by ELISA method and the expression of NLRP3 mRNA was measured by qRT-PCR. Results The level of NLRP3 mRNA and the concentration of IL-1β and IL-18 in plasma of patients with AECOPD or stable COPD was significantly higher than that of the control group (P 〈 0.05 ). The level of NLRP3 mRNA and the concentration of IL-1β and IL- 18 in plasma of the AECOPD group was obviously higher than that of the stable COPD group (P 〈0. 05). The level of NLRP3 mRNA and the concentration of IL-1β and IL-18 in plasma of the respiratory failure group was notably higher than that of the without respiratory failure group ( P 〈 0. 05 ). The correlation was positive between the level of NLRP3 mRNA and the concentration of IL-1β and IL-18, and the correlation coefficient r equal to 0. 69 and 0. 70, respectively (P 〈0. 05). The correlation was positive between the concentration of IL-1β and IL-18 (r =0. 65, P 〈 0. 05). Conclusion NLRP3 and IL-1β, IL-18 involve in the inflammatory reaction of COPD, and the increase of the concentration can cause a serious increase in the severity of the disease.
