快速康复外科在腹腔镜胆囊手术疼痛管理中的应用分析

摘　　要：目的探讨快速康复外科(FTS)在腹腔镜胆囊手术疼痛管理中的应用价值。方法将120例接受腹腔镜胆囊切除术患者作为研究对象,随机分为观察组和对照组,每组60例。观察组行FTS护理,对照组行常规护理。记录两组术后肛门排气时间、下床时间、输液量及进流质饮食时间,比较两组VAS疼痛评分及疼痛严重程度。结果观察组术后1 d、3 d及1周时VAS评分均显著低于术前1 d和对照组差异均有统计学意义(P<0.05)。观察组术后疼痛程度低于对照组,差异有统计学意义(P<0.05)。观察组术后肛门首次排气时间、下床时间、输液量、进流质饮食时间及术后并发症发生率均低于对照组,差异有统计学意义(P<0.05)。结论 FTS用于腹腔镜胆囊手术术后护理能迅速改善疼痛症状,减轻疼痛程度,且有助于患者胃肠功能恢复,缩短康复时间。ObjectiveTo investigate the application value of fast track surgery（FTS） in the pain management of laparoscopic gallbladder surgery. MethodsA total 120 patients underwent laparoscopic cholecystectomy were randomly divided into observation group and control group, 60 cases in each group.The observation group received FTS nursing, and the control group received routine nursing care.The postoperative anal exhaust time, bed time, transfusion volume and the time of liquid feeding were recorded, and the VAS scores and pain degrees were compared. ResultsThe VAS scores of the observation group in 1 day, 3 days and 1 weeks after operation were significantly lower than those before operation and those of the control group in the same time（P＜0.05）.The degrees of postoperative pain in the observation group were significantly lower than those in the control group（P＜0.05）.The postoperative anal exhaust time, bed time, the amount of infusion, the time of feeding and the incidence of postoperative complications in the observation group were significantly shorter than those in the control group（P＜0.05）. ConclusionFTS can rapidly improve the symptoms of pain after laparoscopic cholecystectomy,and it contribute to the recovery of gastrointestinal function and shorten the recovery time.
