摘 要:目的:研究丹参酮胶囊对大鼠同种异体肝脏移植后P_(450)亚型酶及肝功能的影响,分析术后缺血再灌注肝损伤的防护机制。方法:SD大鼠64只,随机分为对照组、模型组、丹参酮胶囊小、大剂量组,除对照组外其余3组大鼠均采用改良双袖套法建立大鼠同种异体肝移植模型,丹参酮胶囊小、大剂量组大鼠分别用1%和2%丹参酮胶囊溶液灌胃3W,对照组及模型组大鼠灌等体积生理盐水。3 W后取静脉血检测大鼠肝功能五项、血清总超氧化物歧化酶(T-SOD)、白细胞介素-6(IL-6)、前列腺素E2(PGE2)水平;取大鼠肝脏检测肝微粒体P_(450)亚型酶活性。结果:丹参酮胶囊小、大剂量组大鼠T-SOD明显提高、CYP2E1、CYP2C9酶活性、IL-6、PGE2及r-GT、TBA、TBil明显下降,存活率分别达56.25%及62.5%,与模型组比较,P<0.05,差异有统计学意义。结论:丹参酮胶囊可提高同种异体大鼠肝脏移植术后T-SOD活性,促进氧自由基清除酶清除氧自由基、降低炎症因子IL-6、PGE2刺激机体造成的炎症反应,并通过抑制P_(450)亚型酶活性来减轻药物代谢时对肝脏的毒性,其活血化瘀的药理特性可增加移植肝脏组织供氧,促进肝功能恢复,提高肝脏移植存活率。Objective:To study the effects of Salvia miltiorrhiza Capsule on ischemia reperfusion injury after liver transplantation in rats and to analyze its protective mechanism. Methods :64 male SD rats were randomly divided into control group, model group, tanshinone cap- sule low and high dose group, except the control group were treated with modified double cuff method to establish rat orthotopic liver trans- plantation model, tanshinone capsule low and high dose group were used 1% and 2% of tanshinone capsule solution by gavage for 3 weeks (W), control group and model group were given equal volume of saline. After 3 W, liver function, serum total superoxide dismutase (T- SOD), interleukin-6 (IL-6), prostaglandin E2 (PGE2) level and liver microsomal P450 subunit activity were detected in the rat liver. Re- suits:compared with the model group, The T-SOD in tanshinone capsule low, high dose group increased significantly;CYP2E1, CYP2C9 en- zyme activity, IL-6, PGE2, GGT, total bile acid (TBA), STB decreased significantly, the survival rate were 56.25% and 62.5%, respec- tively, the difference was statistically significant ( P 〈 0.05 ). Conclusion : Tanshinone capsule could increase the T-SOD activity after al- lograft orthotopic liver transplantation in rats, promote oxygen free radical scavenging enzyme oxygen free radical removal, reduce inflammato- ry cytokines IL-6 and PGE2 stimulated body.
