人性化护理在行血液透析的肿瘤合并肾功能衰竭患者中的应用

摘　　要：目的探讨人性化护理在行血液透析的肿瘤合并肾功能衰竭患者中的应用效果。方法选取2013年4月至2016年1月间崇州市人民医院收治的82例行血液透析的肿瘤合并肾功能衰竭患者。采用随机数表法分为观察组与对照组,每组41例。观察组患者行人性化护理,对照组患者行常规护理。比较两组患者的心理状况、生活质量、并发症发生情况和护理质量的差异。结果护理后,观察组患者焦虑自评量表(SAS)得分为(37.51±3.19)分,明显低于对照组患者的(52.76±3.92)分;观察组患者抑郁自评量表(SDS)得分为(39.31±4.77)分,明显低于对照组的(52.55±4.71)分,两组组间比较差异均有统计学意义(均P<0.05)。观察组患者的生活质量量表总分为(489.76±43.71)分,明显高于对照组患者的(410.39±31.24)分,两组组间比较差异有统计学意义(P<0.05)。观察组患者的高血压、心力衰竭和高钾血症发生率分别为41.5%、34.2%和12.2%,明显低于对照组患者的75.6%、78.1%和34.2%,两组组间比较差异均有统计学意义(均P<0.05)。观察组患者的护理质量总分为(92.62±5.76)分,两组组间比较明显高于对照患者的(50.94±3.76)分,差异有统计学意义(P<0.05)。结论对行血液透析的肿瘤合并肾功能衰竭患者采用人性化护理能够明显缓解患者焦虑和抑郁情绪,提高患者生活质量,降低患者并发症发生率,同时提高护理工作质量。Objective To explore the application of humanized nursing in cancer patients with renal failure on hemodialysis. Methods In Chongzhou City People＇s Hospital,82 cancer patients with renal failure on hemodialysis were selected from April 2013 to January 2016. According to the random number table method,they were randomly divided into an observation group and a control group,with 41 patients in each group. The control group was given routine nursing care and the observation group was given humanized nursing. Psychologic status,quality of life,incidence of complications and the quality of nursing were compared between the two groups. Results After nursing,SAS score was（ 37. 51 ± 3. 19） for the observation group,which was significantly lower than（ 52. 76 ± 3. 92） of the control group. SDS score was（ 39. 31 ± 4. 77） for the observation group which was lower than（ 52. 55 ± 4. 71） of the control group（ all P〈0. 05）. The overall score of SF-36 in the observation group was（ 489. 76 ± 43. 71）,which was significantly higher than（ 410. 39 ± 31. 24） of the control group（ all P〈0. 05）. The incidence rate of hypertension,heart failure and hypertension in the observation group were 41. 5%,34. 2% and 12. 2%,respectively,which was significantly lower than 75. 6%,78. 1% and 34. 2% of the control group（ all P〈0. 05）. The overall quality of nursing score in the observation group was（ 92. 62 ± 5. 76）,which was significantly higher than（ 50. 94 ± 3. 76） of the control group（ all P〈0. 05）. Conclusion Humanized nursing for cancer patients with renal failure on hemodialysis can significantly alleviate the symptoms of anxiety and depression,improve quality of life reduce the incidence of complications,and improve the quality of nursing.
